| 目的：探讨温阳活血法治疗麦默通（Mammotome）乳腺肿块切除术后出血性并发症（局部血肿/皮下瘀斑）的作用 方法：纳入67例行B超引导下Mammotome乳腺肿块切除术后出现出血性并发症（局部血肿和/或皮下瘀斑）的女性患者，完全随机分组为实验组（34例）和对照组（33例）。实验组予阳和汤加减为主方的益气活血中药煎服每日2次，并弹力绷带加压包扎共2周，对照组仅弹力绷带加压包扎2周。每日用视觉模拟评分（Visual analogue score，VAS）评估患者创面疼痛，并测量每例患者的皮下瘀斑或局部血肿面积。结果：实验组患者自觉局部疼痛时间（天）较对照组明显缩短（4.71 vs 12.06，P=0.000）；实验组皮下瘀斑/局部血肿开始消退时间提前，分别为第4天和第7天；皮下瘀斑/局部血肿完全消退所需时间（天）较对照组明显缩短(12.85 vs 20.85,P=0.000)，差异有统计学意义 结论：阳和汤加减为主方的温阳活血法能治疗Mammotome乳腺肿块切除术后出血性并发症。 |

| Objective: To study therapeutic effect of activating blood together with warming healthy Qi of postoperative hemorrhagic instance in ultrasound guided Mammotome operation of breast lesions. Methods: 67 females observed postoperative ecchymosis and/or hematomas in ultrasound guided Mammotome operation of breast lesions were randomly assigned to be divided into treatment group(34 cases), in which the patients were given modified Xue Fu Zhu Yu Tang as the main prescription combined with bandage Compression for 2 weeks, and control group(33 cases), in which the patients only treated with bandage compression for 2 weeks. The wound pain was valued by Visual analogue score (VAS), size of ecchymosis and/or hematoma(mm2) were recorded per day. Results: The duration of wound pain was shorten in treatment group compared with control group（4.71 vs 12.06，p =0.000）as well as the absorbing of ecchymosis and/or hematoma(12.85 vs 20.85,P=0.000); Furthermore, ecchymosis and/or hematoma were also absorbed earlier (d4 vs d7). Conclusion: Activating blood together with warming healthy Qi with modified Yang He Tang as the main prescription has a significant therapeutic effect on Postoperative hemorrhagic instance in ultrasound guided Mammotome operation of breast lesions. |
