      目的 探讨脂肪乳对急性有机磷中毒所致肝损伤的治疗作用。方法 取SD封闭大鼠48只,随机(随机数字法)分为A,B,C,D四组,予敌敌畏11mg/kg 腹腔注射染毒,注射过程中无药物损失即为染毒成功。A组为盐水对照组,染毒后予生理盐水(5ml/kg )静脉注射;B组为脂肪乳对照组,染毒后予20%脂肪乳(5ml/kg )静脉注射;C组为常规治疗组,染毒后立即予阿托品(10mg/kg)和氯解磷定(40mg/kg)肌肉注射;D组为脂肪乳治疗组,在C组的基础上联合应用20%脂肪乳(5ml/kg)静脉注射。分别于染毒前,染毒后30min,2h,4h取眶下血1ml行ALT,AST检;染毒后24小时取肝脏组织行电镜检查。结果 敌敌畏染毒后较染毒前ALT,AST升高明显,差异有统计学意义(P<0.05);染毒后30min,C,D两组ALT,AST明显低于A组,差异有统计学意义(P<0.05);B组较A组,C,D两组较B组ALT,AST降低,但差异无显著性(P>0.05);染毒后2h及4h,C,D两组较A,B两组, D组较C组,B组较A组ALT,AST差异有统计学意义(P<0.05)。肝组织电镜检查示脂肪乳辅助治疗组较其他三组肝叙事改变明显减轻。结论 急性有机磷中毒对肝脏造成严重损害,脂肪乳联合解磷定和阿托品治疗,减轻有机磷中毒所致的肝损伤。

      Objective To explore therapeutical effect of lipid emulsion to acute liver injury induced by acute organophosphorus poisoning in rats. Methods Take sealant-grade Sprague-Dawley(SD) rats, random 48 only into four groups A, B, C, D: saline control group, lipid emulsion control group, the conventional therapy group and lipid emulsion administration group. 11mg/kg body weight dichlorvos (DDVP) was administrated peritoneal injection in all rats, and then the rats in four groups were respectively treated: with normal saline(5ml/kg) intravenous injection, lipid emulsion (5ml/kg) intravenous injection, atropine(5mg/kg)and pralidoxime chloride(40mg/kg) intramuscular injection, combined use of lipid emulsion(5ml/kg) with atropine and pralidoxime chloride after administrating DDVP peritoneal injection. The ALT,AST in blood was detected before and 30min,2h,4h after poisoning in DDVP; The pathological changes of the liver tissue were observed within following 24 h. Results In groups A, B, C, D the ALT, AST in blood were significantly different when compared with before poisoning in DDVP; In groups C,D the ALT, AST in blood was significantly different when compared with group A 30min after DDVP poisoning(P<0.05);There was no significant difference the ALT, AST in blood between group B and group A, and groups C and D (P>0.05); In groups C,D the ALT, AST in blood was significantly different when compared with group A,B as D compared with C and B compared with A 2h and 4h after poisoning in DDVP poisoning(P<0.05).The electron microscopic findings showed the combined use of lipid emulsion with atropine and pralidoxime chloride obviously lessened the liver histopathologic changes after DDVP poisoning. Conclusion acute organophosphorus poisoning induce liver injury,lipid emulsion Combined lipid emulsion with atropine and pralidoxime chloride can protect liver injury induced by acute organophosphorus poisoning.
