| 【】 目的 探讨射频消融术(RFA)对肝癌患者免疫功能的影响,并与手术治疗做对比。方法 将87例患者分为RFA组(45例,行经皮肝穿刺RFA治疗)和手术组(42例,行肝癌根治术治疗)。流式细胞术检测两组患者外周血中CD3 T、CD4 T、CD8 T、CD56 NK细胞比例和CD4 /CD8 比值的变化;通过细胞C3受体花环形成率(RBC-C3bRR)、红细胞粘附肿瘤细胞花环率(RBC-TRR),红细胞吸附免疫复合物花环形成率(RBC-ICR)实验检测外周血红细胞免疫功能;ELISA法检测血清中Th1型细胞因子(IL-2、IFN-γ、TNF-α)的含量。结果 与术前比较,RFA治疗15天后,CD3 、CD4 T细胞和NK细胞数目,以及CD4 /CD8 比值显著升高(P<0.05);治疗30后,这些指标继续升高。而手术治疗的患者治疗第30天后这些指标才有显著升高(P<0.05)。治疗后30天,两组CD8 T细胞数目较术前均有所下降,但并不显著(P>0.05)。RFA治疗15天后较治疗前RBC-ICR显著下降(P<0.05),RBC-C3bRR、RBC-TRR显著升高(P<0.05),而手术治疗第30后患者红细胞免疫指标才有显著改善(P<0.05)。另外,RFA治疗15天后肝患者外周血血清中IL-2、IFN-γ、TNF-α浓度显著升高(P<0.05),30天后继续升高。虽然手术治疗30天后这些Th1型细胞因子浓度显著升高(P<0.05),但显著低于同期RFA组(P<0.05)。结论 RFA较手术治疗更显著改善肝癌患者机体的淋巴细胞和红细胞免疫功能,促进Th1型细胞因子分泌,提高机体免疫力,从而对肝癌的治疗效果产生有益的影响。 |

| 【】 Objective To explore the effect of RFA on post-operative immune function of hepatic carcinoma patients, and comparing with surgical resection. Methods 87 patients were divided into two groups with 44 treated by radio frequency ablation and 42 treated by surgical resection. The percentage of CD3 T、CD4 T、CD8 T、CD56 NK cells and the ratio of CD4 /CD8 in the peripheral blood of patients were assayed by fIow cytometry. Red blood cell C3b receptor rosette (RBC-C3bRR),tumor red blood cell rosette(RBC-TRR) and red blood cell immune complex rosette(RBC-ICR) were detected to check the immune function of red blood cell. Thl type cytokines (IL-2, IFN-γ, TNF-α) in the sera were detected by ELISA assay. Results Compared to prior-treatment, 15 days after RFA, CD3 , CD4 T, NK cells and CD4 /CD8 ratio increased significantly (P<0.05), 30 days later, these indicators increased further. And these indicators in surgical group increased significantly only after 30 days of treatment (P<0.05). The percentage of CD8 T cells in both groups have decreased after 30 days of treatment, but not significantly (P>0.05). 15 days after RFA, RBC-ICR decreased significantly (P<0.05), and RBC-C3bRR, RBC-TRR increased significantly (P<0.05). And the indicators of immune function of red blood cell improved significantly only after 30 days of treatment (P<0.05). Additionly, IL-2, IFN-γ, TNF-α in the sera increased significantly 15 days after RFA (P<0.05), then they kept on increasing 30 days after RFA. These Thl type cytokines increased significantly 30 days after RFA (P<0.05), but lower than RFA group significantly (P<0.05). Conclusion Compared with surgical treatment, RFA can more significant improve immune function of lymphocytes and red blood cells of hepatocellular carcinoma patients, promote secretion of Th1 type cytokines. And thus having a beneficial effect on the treatment of hepatocellular carcinoma. |
