| 王邦才教授临床以擅治胃病著称。认为通降是胃生理特点的集中体现；滞而不通是胃病理特征的高度概括。是以临床治疗胃病多以“通降”立论。但“通降”之法,老师认为非通腑泄浊一途,当辨寒热虚实不同而分治之,配伍严谨,疗效颇佳。如气滞中脘,胃失和降者,予理气通降法；肝失疏泄,横侮胃土者,当泄肝通胃法；湿热蕴结,食积阻滞者,宜降胃导滞法；寒热错杂,胃脘痞、满、呕、泄并见者,则用辛开苦降法；而对胃阴不足,胃失濡润者,宜甘寒通降和胃法；胃气不足,中阳不振,消磨无力者,尊叶天士之意,用通补阳明法。笔者择其养阴通降和胃,通补胃阳,通腑降逆方面予以不同侧面介绍王师通降立法治疗胃病经验。 |

| Professor WANG Bangcai is well known for the treatment of gastropathology. He proposed that the dredging and descending as physiological characteristics of the stomach while stagnation is an overview of the physical characteristics of the stomach. Therefore, the application of dredging and descending principle is widely used clinically. However, Dr.Wang advocates that the application of this principle is more than just a way of purging fuorgans and excreting the waste. One should distinguish between the cold and heat and deficiency and excess to ensure precise combination of herbs, bringing pronounced clinical efficacy. For example for qi stagnation in the abdomem causing the inability of stomach-qi to descend, it is treated by activating qi and purging fuorgans, for stagnation of liver qi, wood over-acting earth, it is treated by purging liver. For retention of damp-heat or food retention, it is treated by descending the stomach-qi and removing food retention. For simultaneous occurrence of cold and heat syndromses, with symptoms of bloated stomach, vomit and diarrhoea, herbs acrid in flavor for dispersing and herbs bitter in flavor for descending are used. For insufficiency of stomach yin,lack of moisture of the stomach, herbs with sweet and cold property are used to enhance descending of the stomach-qi. For insufficiency of stomach qi, weakening of yang of middlejiao causing indigestion,Professor Wang adheres to the will of YR Tianshi, treating mainly with purging and invigorating YangMing. This article explores Dr. Wang"s application of dredging and descending in the treatment of gastropathy from nourishing yin and dredging and descending, purging and invigorating stomach yang, purging fu-organs and calming the adverse-rising energy, allowing the study of this principle in the treatment of gastropathy from a different level. |
