| 目的 探讨长时血压变异性（BPV）在脑白质疏松（LA）发病中的意义。方法 收集2011年6月至2013年6月于浙江医院就诊的高血压患者209例，每月随诊测血压1次，随访12个月，根据头颅MRI结果采用ARWMC评分法，比较LA组与非LA组以及LA组内不同程度白质疏松患者之间的长时血压变异性指标。结果 LA组长时平均收缩压（135.8±20.1mmHg）、平均舒张压（79.1±15.4mmHg）、收缩压标准差（13.0±2.7）、舒张压标准差(9.7±2.0)、收缩压变异系数（9.1±2.1）和舒张压变异系数（11.9±1.5）均高于非LA组长时平均收缩压（125.4±17.1mmHg）、平均舒张压（74.5±7.9mmHg）、收缩压标准差（9.7±2.2）、舒张压标准差(7.8±1.0)、收缩压变异系数（7.0±1.9）和舒张压变异系数（10.5±1.3）（P<0.05）。不同LA程度患者之间比较，长时平均血压、血压标准差及血压变异性无统计学差异（P>0.05）。结论 长时血压变异性影响LA的发生。 |

| Objective To investigate the relationship between long-term blood pressure variability(BPV) and leukoaraiosis(LA). Methods 209 patients from Zhejiang Hospital between June 2011 and June 2013 were consecutively recruited and followed up 12 months.Blood pressure measurements were taken at every interview. The patients were divided according to ARWMC scale,and compared long -term blood pressure and the index of long-term BPV among LA group ,non-LA group and every level group in LA group.? Results The systolic blood pressure(SBP) in LA group was 135.8±20.1mmHg,and SBP in non-LA group was 125.4±17.1mmHg.The diastolic blood pressure(DBP) in LA group and in non-LA group were 79.1±15.4 mmHg and 74.5±7.9 mmHg. The standard deviation of SBP in LA and non-LA group were 13.0±2.7 and 9.7±2.2,and what of DBP in LA and non-LA group were 9.7±2.0 and 7.8±1.0.The coefficient?of variation of SBP and DBP in LA group were 9.1±2.1 and 11.9±1.5,and what in non-group were 7.0±1.9 and 10.5±1.3.The analysis showed that the average of long-term blood pressure,the standard deviation (SD) and?coefficient?of variation (CV) in LA group were higher than non-LA group(P<0.05) . There were no statistical differences among the range of severity?of LA (P>0.05). Conclusion This study indicates that long-term blood pressure variability affect the incidence of leukoaraiosis. |
