| 目的 观察盐酸羟考酮用于蛛网膜下腔麻醉下痔上粘膜环切术(PPH)的临床效果。方法 随机选择60例ASA Ⅰ-Ⅱ级的PPH术患者分为两组,A组为实验组,在麻醉平面稳定后静注3mg(稀释到3ml)盐酸羟考酮,B组为对照组,静注3ml生理盐水,记录两组患者手术前及给药后5分钟、环切时的心率、无创平均动脉压和血氧饱和度的变化,统计恶心、呕吐、呼吸抑制、心动过缓等药物不良反应的发生例数。结果 A组内脏牵拉反应明显轻于B组(P ＜ 0.05),A组比B组能更好地维持生命体征的平稳,并降低恶心、呕吐、心动过缓的发生率(P ＜ 0.05),两组均无呼吸抑制发生。结论 静注盐酸羟考酮3mg对抑制蛛网膜下腔麻醉下PPH术的内脏牵拉反射效果确切, 呼吸、循环维持稳定,安全可靠。 |

| Objective To evaluate the clinical effectiveness of oxycodone hydrochloride under Procedure for Prolapse and Haemorrhoids (PPH) with spinal block. Methods 60 ASAI or II PPH patients were randomly selected, and were randomly divided into two groups, which named group A and group B. Group A is oxycodone hydrochloride injection group which intravenous injection with 3mg oxycodone hydrochloride (0.9% saline diluted to 3ml) after level of anesthesia stabled , and group B is saline control group which intravenous injection with 3ml saline after level of anesthesia stabled. Heart rate before surgery, 5 minutes after injection and when using circular stapler were recorded for each group. And changes in noninvasive mean arterial blood pressureand oxygen saturation were also recorded. Simultaneously counted the cases of adverse reaction about nausea, vomiting, respiratory depression, bradycardia etc. Results Statistics show that respiratory depression didn’t occurred in both groups. Compared to group B, group A can maintain vital signs more steady, and reduce the probability of nausea , vomiting and bradycardia (P<0.05). And patients had less visceral traction response in group A (P <0.05). Conclusion Intravenous injection with 3mg oxycodone hydrochloride for visceral traction response in patients undergoing PPH with spinal block is effective and safety with steady breathing and systemic circulation. |
