      目的 探讨彩色多普勒超声在乳腺癌新辅助化疗疗效评估中的应用价值。方法 选择66例行新辅助化疗的乳腺癌患者,根据化疗效果分为化疗有效组与化疗无效组,分别在新辅助化疗前与化疗后手术前行彩色多普勒超声检查。观察肿瘤的大小、边界、形态、内部回声、后方回声及内部血流情况,并比较并分析观察指标与新辅助化疗疗效之间的联系。结果两组患者化疗后乳腺肿瘤体积均较治疗前明显缩小(P<0.05或P<0.01),化疗有效组缩小的幅度明显大于化疗无效组(P<0.01);化疗有效组化疗后乳腺肿瘤边界、形态、内部回声及后方回声与化疗前及化疗无效组化疗后比较均有显著差异(P<0.05或P<0.01);化疗有效组化疗后血流信号丰富例数明显少于化疗前与化疗无效组化疗后(P<0.05或P<0.01)。结论 采用彩色多普勒超声检测乳腺癌患者,通过比较二维及彩色多普勒表现,可以有效的评价乳腺癌新辅助化疗的疗效,为临床的下一步治疗提供重要的依据。

      Objective: To discuss application value of color Doppler ultrasonography on curative effect evaluation of neoadjuvant chemotherapy in mammary cancer. Methods: 66 cases of patients with mammary cancer, who were treated by neoadjuvant chemotherapy, were selected and divided into effective chemotherapy group and ineffective chemotherapy group in accordance with the chemotherapy results, and given color Doppler ultrasonography examination before and after neoadjuvant chemotherapy and operation. The size, boundary, shape, internal echo, rear echo and internal blood flow of tumor were observed, and the relationship between observation index and curative effect of neoadjuvant chemotherapy were compared, analyzed and observed. Results: The volumes of mammary cancer of patients in two groups after chemotherapy were obviously reduced than before (P<0.05 or P<0.01), and the reducing rate of patients in effective chemotherapy group was much higher than that in ineffective chemotherapy group (P<0.01). The boundary, shape, internal echo and rear echo of mammary tumor of patients in effective chemotherapy group after chemotherapy showed significant differences than before and those in ineffective chemotherapy group after chemotherapy (P<0.05 or P<0.01). The rich cases of blood flow signals of patients in effective chemotherapy group were much less than before and those in ineffective chemotherapy group after chemotherapy (P<0.05 or P<0.01).Conclusion: By comparing the performance of two dimensional ultrasonography and color Doppler ultrasonography, color Doppler ultrasonography can effectively evaluate the curative effect of neoadjuvant chemotherapy in mammary cancer, which provides significant evidence for the further clinical treatment.
