| 目的：观察银质针导热配合热罨包外敷治疗肾阳虚型膝骨关节炎的临床疗效。
方法：选择膝骨关节炎患者150例，抽签法随机分为3组，即观察组50例采用银质针导热疗法配合热罨包外敷，对照1组50例采用银质针导热疗法，对照2组50例采用透明质酸钠关节腔内注射，治疗结束后，分别观察治疗前后HSS膝关节评分、临床疗效及关节滑液中IL-1β、IL-6、INF-α的变化。
结果：各组患者膝关节的疼痛评分、功能评分、屈曲畸形评分、稳定性评分、活动度评分、减分项目及总评分，治疗后观察组明显高于对照2组（P<0.05，P<0.01）；改善程度，观察组明显高于对照2组（P<0.01）。关节肌力评分及改善程度，治疗后，各组无明显差异（P>0.05）；各组优良率、有效率，对照2组虽低于对照1组、观察组，但无显著性差异（P>0.05）。各组关节液中IL-1β、IL-6、TNF-α含量，治疗前后有明显差异（P<0.01）；其中IL-1β、IL-6、TNF-α治疗后观察组明显低于对照2组（P<0.01），对照1组低于对照2组（P<0.05，P<0.01）。
结论：银质针导热疗法配合热罨包外敷的综合疗法能有效缓解肾阳虚型膝骨关节炎的临床症状。 |

| ABSTRACT
OBJECTIVE：To observe the clinical efficacy of thermal conductivity of silver needle combined with external application of heat foment package treatment of kidney-yang deficiency type of knee osteoarthritis.
METHODS：Select patients with knee osteoarthritis 150 cases were randomly divided into three groups. Observation group of 50 patients with thermal conductivity of silver needle combined with external application of heat foment package, control group 1 of 50 patients with thermal treatment of silver needle, and control group 2 of 50 patients with intra-articular injection of sodium hyaluronate. After treatment, each group was observed HSS knee score, clinical and synovial fluid in the IL-1β, IL-6, TNF-α change of before and after treatment.
RESULTS：Pain score, function score, flexion deformity score, stability score, activity score, by sub-item and total score of Knee in each group, after treatment, Observation group was significantly higher than the control group 2 (P <0.05, P <0.01); the degree of improvement of symptoms, observation group was significantly higher than the control group 2 (P <0.01). After treatment, joint muscle strength score and the degree of improvement in each group no significant difference (P> 0.05). The group good and efficiency rate, control group 2 although lower than the control group 1 and the observation group, but no significant difference (P> 0.05). Synovial fluid of each group IL-1β, IL-6, TNF-α levels before and after treatment were significantly different (P <0.01); after treatment, IL-1β, IL-6 and TNF-α of the observation group were significantly lower than that of the control group 2 (P <0.01) and the control group 1 was significantly lower than the control group 2 (P <0.05, P <0.01).
CONCLUSION: The thermal conductivity of silver needle combined with external application of heat foment package of combination therapy could effectively alleviate the clinical symptoms of kidney-yang deficiency type of knee osteoarthritis. |
