| 目的 回顾性分析甲状腺微小乳头状癌(PTMC)和甲状腺良性结节中人骨髓内皮细胞标记物(HBME-1)、半乳糖凝集素3(Galectin-3)及高分子量角蛋白(34βE12)的表达差异,并探讨其鉴别诊断的临床价值。方法 采用免疫组化方法检测并比较189个PTMC病灶及106例甲状腺良性结节中HBME-1、Galectin-3及34βE12的表达水平,计算其对PTMC的诊断效能指标。结果 HBME-1、Galectin-3及34βE12在PTMC中的表达阳性率显著高于甲状腺良性疾病组(均P<0.001)。HBME-1、Galectin-3及34βE12诊断PTMC的敏感度分别为89.95%、95.23%、93.65%,特异度分别为83.02%、71.70%、81.13%,准确度分别为87.45%、86.78%、89.15%,ROC-AUC分别为0.891、0.914、0.932。结论 HBME-1、Galectin-3及34βE12在PTMC中的表达显著高于良性病变,具有较高的诊断价值,可以作为诊断PTMC的重要标记物。 |

| Objective To retrospectively analyze the expressions of HBME-1,Galectin-3 and 34βE12 in Thyroid nodules,and to explore its diagnostic value for papillary thyroid microcarcinoma (PTMC). Methods Immunohistochemical technique was performed to detect the expression of HBME-1,Galectin-3 and 34βE12 in 295 thyroid lesions.The receiver operating characteristic area under the curve (ROC-AUC) and their index for diagnosis evaluation were also calculated. Results It was statistically found that there were strikingly significant differences for the three markers in the distinguishing diagnosis of malignant and benign thyroid nodules(All P<0.001). For a single analyzed tumor marker,the sensitivity of the above three markers was 89.95%,95.23%,93.65%,respectively. The specificity was 83.02%,71.70%,81.13%,respectively.The diagnostic accuracy rate was 87.45%,86.78%,89.15%,and the ROC-AUC of three markers was 0.891,0.914,0.932,respectively. Conclusion The expressions of HBME-1,Galectin-3 and 34βE12 in PTMC lesions are significantly higher than those in benign nodules with the perfect diagnostic parameters,indicating that they are ideal biomarkers for PTMC. |
