      目的:通过对急性脑出血模型兔动脉血可溶性血栓调节蛋白(sTM)、血管性假性血友病因子(vWF)以及脑组织标本病理变化的观察,探讨乌司他丁(UTI)在急性脑出血方面的脑保护作用。方法:将造模成功的30只兔随机平均分为三组,每组10只:脑出血模型组(M组),依达拉奉治疗组(Y组),乌司他丁治疗组(U组)。应用ELISA法测定各组兔急性脑出血后动脉血中sTM、vWF的含量,以及兔脑出血病灶周围脑组织病理变化。结果:与M组[sTM(24h:6.06±0.19ng/ml,72h:6.42±0.48 ng/ml);vWF(24h:10.02 ±0.71ng/ml,72h:11.39±0.63ng/ml) ]相比,Y组vWF含量在24h[9.77±0.46ng/ml] 无明显统计学差异(P>0.05),但其在72h [4.51± 0.10ng/ml] 时下降,有统计学差异(P<0.05);Y组的sTM含量在24h[4.61±0.11ng/ml]、72h[4.51±0.10ng/ml]均有下降,有统计学差异(P<0.05);U组的sTM、vWF含量在24h[sTM(4.82±0.77 ng/ml), vWF(9.09±0.24 ng/ml)] 、72h [sTM (4.79±0.75ng/ml), vWF (8.48±0.73ng/ml)] 均下降,有统计学差异(P<0.05)。与Y组[sTM (24h:4.61±0.11ng/ml),72h:4.51± 0.10ng/ml);vWF (24h:9.77±0.46ng/ml,72h:9.64±0.56ng/ml)]相比,U组sTM含量24h [ 4.82±0.77ng/ml]、72h[ 4.79±0.75 ng/ml] 均无统计学差异(P>0.05);U组vWF含量24h [24h: 9.09± 0.24ng/ml、72h:8.48 ±0.73 ng/ml]均有统计学差异(P<0.05)。病理结果提示:U组脑出血病灶周围脑细胞的线粒体嵴断裂、胞浆内空泡增多、细胞肿胀方面均比Y组延缓。结论:乌司他丁可以降低急性脑出血模型兔血清sTM及vWF含量,可以延缓出血灶周围脑细胞线粒体及细胞核的损害,具有一定的脑保护作用。

      Objective To observe acute cerebral hemorrhage model in rabbit arterial blood soluble Thrombomodulin (sTM), von Willebrand Factor (vWF) and brain tissue specimens of pathological changes, we study the effect of ulinastatin(UTI)on protective of the brain. Methods Thirty successful Modeled rabbits were randomly divided into 3 groups in average: intracerebral hemorrhage (ICH) model group (M group), edaravone treatment group (Y group), UTI treatment group (U group),10 rabbits each group. ELISA method was used to determine the content of vWF and sTM in the arterial blood; and to observe the pathological changes of the brain tissues around acute cerebral hemorrhage in the rabbits. Results: Compared with M group [sTM(24h:6.06±0.19ng/ml,72h:6.42±0.48 ng/ml); vWF (24h:10.02 ±0.71ng/ml, 72h:11.39±0.63ng/ml)], content of vWF in Y group at the 24h[9.77±0.46ng/ml] point had no significant statistical difference (P > 0.05),but it at the 72h[4.51±0.10ng/ml] point had significant statistical difference (P<0.05); content of sTM in Y group at the 24h[4.61±0.11ng/ml] and 72h[4.51±0.10ng/ml] point both had statistical difference (P<0.05);content of sTM and vWF in U group at the 24h[sTM(4.82±0.77 ng/ml), vWF(9.09±0.24 ng/ml)] and 72h [sTM (4.79±0.75ng/ml), vWF (8.48±0.73ng/ml)] point all had significant statistical difference (P<0.05). Compared with Y group [sTM (24h:4.61±0.11ng/ml), 72h:4.51±0.10ng/ml); vWF (24h:9.77± 0.46ng/ml, 72h:9.64±0.56ng/ml)], content of sTM in U group had no significant difference on 24h [ 4.82±0.77ng/ml] and 72h[4.79±0.75 ng/ml] piont (P>0.05); content of vWF in U group [24h: 9.09± 0.24ng/ml, 72h:8.48 ±0.73 ng/ml] were significantly different from that in Y group (P<0.05). Pathological results: the cells around the lesion of ICH,appear mitochondria crista broken and cytoplasmic vacuoles increased and cell swelling,but those in U group were obviously lightly than Y group. Conclusion UTI can reduce acute ICH model rabbit serum content of sTM and vWF, can delay the damage of surrounding focal hemorrhage brain mitochondria and the nucleus, has a certain brain protection.
