| 目的 观察低温血液透析联合左卡尼汀对糖尿病肾病患者透析中低血压的影响。方法 选择23例易发生透析中低血压的糖尿病肾病患者,采用前瞻性自身配对研究,先后给予常规血液透析(A组)、低温血液透析(B组)、低温血液透析联合左卡尼汀(C组)各3个月。观察患者透析中低血压的发生情况,以及血肌酐、尿素氮、白蛋白、空腹血糖、血红蛋白、电解质及干体质量和超滤量的变化。结果 B组与C组的透析中低血压发生率低于A组(P＜0.05),而且C组的透析中低血压发生率明显低于B组(P＜0.05)。但三组的透析前血压、血肌酐、尿素氮、白蛋白、空腹血糖、血红蛋白、电解质及干体质量和超滤量差异无统计学意义(P＞0.05)。结论 应用低温血液透析联合左卡尼汀可有效降低糖尿病肾病患者透析中低血压的发生率,且无明显不良反应。 |

| 【】Objective To observe the effect of low-temperature hemodialysis combined with levocarnitine on the intradialytic hypotension(IDH) in patients with diabetic nephropathy. Methods Twenty-three diaberie nephropathy patients with a prior history of IDH were enrolled. All patients were given routine hemodialysis (group A),low-temperature hemdialysis (group B) and low-temperature hemdialysis combined with levocarnitine(group C) in succession respectively three months (using self-match pair study). The change of the occurrence of IDH were compared. Meanwhile, the changes of blood tests including serum creatinine (Scr), blood urea nitrogen (BUN), albumin(Alb), fasting plasma glucose (FPG), hemoglobin (Hb), concentration of K, Na, Ca, P, dry weight, ultrafiltration volume were recorded an danalyzed. Results The incidence of IDH in group B and group C was lower than that in group A (P < 0.05), and the incidence of IDH in group C was significantly lower than that in group B (P < 0.05). But there were not significant differences in blood pressure before hemodialysis, blood tests, dry weight and ultrafiltration volume between the 3 groups (P＞0.05). Conclusions Low-temperature hemdialysis combined with levocarnitine can effectively decreased the incidence of IDH in patients with diabetic nephropathy and has no significant adverse reactions. |
