| 目的 比较分析手法闭合复位与切复内固定治疗青少年桡骨小头的临床疗效。方法 2005年6月至2013年6月在本院或朱氏中医骨伤科门诊部采用手法闭合复位或切复内固定治疗并获得完整随访的青少年桡骨小头骨折36例,其中手法闭合复位组15例,男9例,女6例,年龄6~23岁,平均(14.33±4.39)岁,按照Mason分型：Ⅱ型8例,Ⅲ型7例；切复内固定组21例,男14例,女7例,年龄8~22岁,平均(14.86±3.92)岁；按照Mason分型：Ⅱ型9例,Ⅲ型12例。观察两组患者治疗前后的桡骨移位距离,并随访采用Broberg and Morrey肘关节功能评分评价肘关节功能及采用Metaizeau肘关节功能后期疗效标准评定治疗效果并进行比较分析。结果手法闭合复位组和切复内固定组患者在治疗前的桡骨移位距离相互比较无明显差异(P>0.05),治疗后3d两组患者的桡骨移位距离分别为(0.99±0.57)mm和(0.57±0.28)mm,均明显小于术前移位距离(P<0.01),并且手法闭合复位组的Broberg and Morrey肘关节功能评分为(86.93±10.85),明显高于切复内固定组(P<0.05),手法闭合复位组的Metaizeau疗效评价中总较好率为86.67%、不良率为6.67%,与切复内固定组比较均无明显差异(P>0.05)。结论 手法闭合复位与切复内固定治疗青少年桡骨小头一样具有良好的临床疗效,并且能够更好的恢复肘关节功能。 |

| Objective: To research the clinical effects of Closed manual reduction and Open reduction and internal fixation in the treatment of teenagers radial head fracture. Methods: The complete follow-up of adolescent patients with radial head fracture were 36 cases and were treated by Closed manual reduction and Open reduction and internal fixation in our hospital or Traumatology Clinics of Zhu from June 2005 to June 2013. The closed manual reduction group have 15 caes, including 9 males and 6 females, average age of (14.33±4.39)years, fractures type by Mason classification including 8 cases with II types fractures and 7 cases with III types fractures. The open reduction and internal fixation group have 21 caes, including 14 males and 7 females, average age of (14.86±3.92)years, fractures type by Mason classification including 9 cases with II types fractures and 12 cases with III types fractures. The distance of radial displacement of two groups were compared in before and after treatment. The elbow function of two groups were evaluated by Broberg and Morrey elbow function score and the efficacy were assessed by Metaizeau curative effect evaluation of elbow function. They were compared and analyzed each other. Results: The distance of radial displacement of Closed manual reduction and Open reduction and internal fixation were not significant differences before the treatment (P>0.05). The distance of radial displacement of two groups were (0.99±0.57) mm and (0.57±0.28) mm, they were less than before treatment, they were significant differences compared with before treatment (P<0.05). The Broberg and Morrey elbow function score of Closed manual reduction was (86.93±10.85), it was significantly higher than the Open reduction and internal fixation, and they were significant differences (P<0.05). The Metaizeau curative effect evaluation The general good rate and Defective rate of Closed manual reduction were 86.67% and 6.67% by the Metaizeau curative effect evaluation, they were not significant differences compared with Open reduction and internal fixation (P>0.05). Conclusion: The Closed manual reduction is a effective treatment of teenagers radial head fracture, it have similar clinical efficacy with Open reduction and internal fixation, and it is better to restore elbow function. |
