      目的:观察丹参多酚酸盐对糖尿病(DM)合并急性心梗PCI术后心肌微循环的影响。方法:将56例糖尿病合并ST段抬高型急性心肌梗死(AMI)患者随机分为两组,治疗组28例,予以直接PCI术 常规药物 丹参多酚酸盐,对照组28例,仅予以直接PCI术 常规药物治疗,比较治疗前后两组间心电图、心肌酶谱、并发症、心功能等相关指标差异。结果:(1)2组患者PCI术后即刻的TIMI血流分级、CTFC值无明显差异(P>0.05)。(2)治疗后第2、24小时治疗组ST段下降幅度、QTd间期均短于对照组,差异具有统计学意义(P<0.05)。(3)治疗7天后两组BNP、LVEF及CO较治疗前均有明显改善,治疗组较对照组改善更为明显(P<0.05);两组LVEDD均无明显变化(P<0.05)。(4)与对照组相比治疗组CK-MB酶峰前移并明显下降(P<0.05)。结论:丹参多酚酸盐可以提高DM合并AMI患者PCI术后的治疗疗效,有效促进DM合并AMI患者的心肌再灌注,减少心肌损伤和梗死面积,改善心功能。

      Objective To observe the influence of percutaneous coronary interventions (PCI) combined with polydanshinolate on myocardial reperfusion in DM patients with acute ST-elevation myocardial infarction.Methods A total of 56 cases patients with AMI were randomly divided into control group and observation group with 28 cases in each group, The control group was treated with PCI combined with polydanshinolate and regular medicine,the observation group was treated with PCI and regular medicine.The indexes of coronary microcirculation of two groups were compared.Results There was no difference in TIMI and CTFC between two groups after operation (P>0.05). After 2,24 hours of treatment,the ST segment of control group declined more significantly and QTd interval was shorter than observation group(P<0.05). After one week of treatment, LVEF,CO of two groups were all increased,whereas BNP decreased,there is a significant change in control group (P<0.05). No significant difference of LVEDD was found (P>0.05). Compared with the observation group there were significantly lower CK-MB peak and earlier time of CK-MB peak in control group (P<0.05). Conclusions Polydanshinolate addition to PCI could reduce no reflow phenomenon in DM patients of acute myocardial infraction and improve myocardial reperfusion.
