| 目的：探讨电针对行机械通气重症急性胰炎患者的的镇静作用。方法：选择30例入住重症医学科需要行经口气管插管机械通气的重症急性胰腺炎患者,随机分为2组。对照组：采用单纯静脉注射咪达唑仑镇静；治疗组：电针刺激神庭、印堂穴加静脉注射咪达唑仑镇静。通过调整咪达唑仑微泵泵速,控制脑电双频指数为60-80分。观察两组患者镇静6小时的咪达唑仑用量差异。结果：两组患者的6小时的咪达唑仑的平均用量分别为对照组为0.08±0.03mg/Kg.h, 治疗组为0.05±0.02mg/Kg.h, 治疗组咪达唑仑用量较对照组相比有显著下降(P<0．05)。在镇静开始时(第1h)两组间的咪达唑仑用量差异无统计学意义(P＞0.05)；在第2ｈ～６ｈ治疗组的咪达唑仑使用剂量与对照组相比有显著减少,其差异有统计学意义(P均<0.05)。结论：电针对行机械通气的重症急性胰腺炎患者进行镇静可减少咪达唑仑的用量,进而表明针刺神庭、印堂穴并留针加电刺激对行机械通气的危重症患者有一定的镇静作用。 |

| Objective To investigate the sedative effect of electro-acupuncture stimulation and Midazolam on patients with severe acute panreatitis(SAP) during mechanical ventilation(MV). Methods 30 patients with SAP during MV were included and divided into two groups randomly. Intravenous injection of midazolam was given to patients in control group. In patients in treatment group,midazolam was given also and acupuncture at Shenting(DU24)and Yintang (EX-HN3)with needle retention and electroacupuncture was applied simultaneously. The speed of intravenous injection of Midazolam was controlled through micro-pump to maintain the bispectral index(BIS) between 60 and 80．Then , we observed the dose of Midazolam used. Results The hourly mean dose of midazolam used within the first six hours after sedation was(0.08±0.03)mg/kg.h in control group, and (0.05±0.02)mg/kg.h in treatment group respectively(P＜0.05). The hourly dose of midazolam were found at every hour from 2 to 6 hours after sedation(all P＜0.05). Conclusion Sedation of electro-acupuncture stimulation and Midazolam can decrease the dose of Midazolam used, therefore ,electro-acupuncture at Shenting and Yintang has a certain sedative effect on patients with SAP during MV. |
