| 目的分析临床触诊阴性乳腺癌的X线、超声的影像特点,与病理的关系以及X线、超声在诊断中的作用。方法 通过96例患者97个病灶,计算X线、超声的敏感性;分析X线、超声的影像与病理的关系。结果对临床触诊阴性的乳腺癌,乳腺X线的敏感性为62.4%(58/93);乳腺超声的敏感性为52.7%(49/93);乳腺X线+超声的敏感性为87.1%(81/93)。乳腺X线发现的58例58个病灶的乳腺X线表现主要为钙化灶74.1%(43/58),其病理主要为原位癌,占65.1%(28/43);其次为结节或肿块影15.5%(9/58),其病理主要为浸润性癌,占77.8%(7/9)。乳腺超声发现的48例49个病灶表现主要为低回声结节55.1%(27/49),其病理主要为浸润性癌,占66.7%(18/27);其次表现为偏低回声区36.7%(18/49),其病理主要为原位癌,占77.8%(14/18)。结论乳腺X线和超声联合可以发现87.1%的触诊阴性的乳腺癌; 乳腺影像与病理有相应的关系;乳腺X线、超声及仔细的体格检查可以发现更多、更早期的触诊阴性的乳腺癌。 |

| Objective: To explore feature of mammography, ultrasound and pathology of non-palpable breast carcinoma, to identify diagnostic effect of mammography and ultrasound. Method: Calculate sensitivity of mammography and ultrasound, analyse relationship between imaging and pathology through 97 lesions in 96 patients. Result:For non-palpable breast carcinoma, sensitivity of mammography was 62.4%(58/93), sensitivity of ultrasound 52.7%(49/93), sensitivity of mammography and ultrasound was 87.1%(81/93) We found calcification 74.1%(43/58)by mammography, most of them were carcinoma in situ 65.1%(28/43);We also found nodus or mass 15.5%(9/58) by mammography ,most of them were invasive carcinoma 77.8%(7/9).We found hypoechoic nodule 55.1%(27/49)by ultrasound, most of them were invasive carcinoma 66.7%(18/27);We also found hypoechoic region 36.7%(18/49) by ultrasound, most of them were carcinoma in situ 77.8%(14/18).Conclusion: We could find 87.1% of non-palpable breast carcinoma by combination of mammography and ultrasound. The pathology of the non-palpable breast carcinoma was related to the image of them. Throught mammography , ultrasound and physical examination could find more earlier non-palpable breast carcinoma. |
