| 目的：探讨儿童传染性单核细胞增多症并发肝损害的临床和免疫特点。方法：将我院收治的68例急性感染期传染性单核细胞增多症患儿分为肝损害组（38例）和非肝损害组（30例），比较两组患儿的年龄分布、临床症状、体液免疫、补体和细胞免疫特点；结果：肝损害组患儿肝肿大发生率高于非肝损害组，差异有统计学意义（P<0.05）。肝损害组患儿的IgA明显高于非肝损害组，差异有统计学意义（P<0.05）。肝损害组患儿的CD4+/CD8+比值明显低于非肝损害组，差异有统计学意义（P<0.05）。结论：儿童传染性单核细胞增多症并发肝损害组与非损害组相比，肝肿大的发生率高，IgA水平明显升高，CD4+/CD8+比值明显降低；当患儿肝功能受损时，应及时采取有效治疗，减少并发症发生。 |

| Objective To explore the clinical and immunological characteristics of children infectious mononucleosis complicated with hepatic lesion. Methods 68 children with infectious mononucleosis in our housptial,based on whether they were complicated with hepatic lesion, were allocated to hepatic lesion group (38 patients) and non-hepatic lesion group (30 patients). Characteristics of age distribution, clinical symptoms, humoral immunity, complement and cell immunity were compared between the two groups. Results Incidences of hepatomegaly in the hepatic lesion group were higher than those in the non-hepatic lesion group, the differences were significant ( P<0.05);The levels of IgA in the hepatic lesion group were higher than those in the non-hepatic lesion group, the differences were significant ( P<0.05);The ratio of CD4+/CD8+ in the hepatic lesion group was lower than those in the non-hepatic lesion group, the differences were significant ( P<0.05); Conclusion The hepatic lesion group compared with the non-hepatic lesion group,hepatomegaly incidence and IgA levels were significantly increased, the ratio of CD4+/CD8+ was significantly reduced;when the patient complicated with hepatic lesion，we should take timely effective treatment to reduce complications. |
