| 【】 目的 研究完全腹腔镜胃癌根治术(TLG)的可行性,安全性和临床疗效。方法 对2014年6月至2015年6月在金华市中心医院进行完全腹腔镜胃癌根治术的58例患者的临床资料进行回顾性分析。 结果 58例均顺利完成TLG,无一例中转开腹手术。术后2例发生十二指肠残端漏,保守治疗后好转。本组病例中远端胃大部切除45例,采用胃十二指肠三角吻合20例,采用毕Ⅱ式胃空肠吻合25例；全胃切除13例,均采用食管空肠侧侧吻合。手术时间160-340min,平均(220±35)min。术中出血60-350ml,平均(125±50)ml。清扫淋巴结18-40枚,平均(26±6)枚。无围手术期死亡。术后住院时间7-12 d,平均(9±2)d。术后随访6-12个月,未见复发。结论 完全腹腔镜胃癌根治术是安全可行的。 |

| 【】Objective To investigate the feasibility, safety and clinical efficacy of totally laparoscopic gastrectomy(TLG)for gastric cancer. Methods The investigators retrospectively analyzed 58 cases undergoing TLG for gastric cancer from June 2014 to June 2015 at Jinhua Central Hospital．Results All 58 cases underwent successful TLG．There was neither transfer to open．Two cases had post-operative complications,with 2 case of Duodenal stump leakage recovering well after conservative treatment.45 cases underwent distal gastrectomy,including 20 cases with delta-shaped gastroduodenostomy and 25 cases with Billroth II reconstruction; 13 cases total gastreetomy with side-to-side esophageal jejunostomy．the operation duration was 160-340min[mean(220±35) min]．The blood loss was 60-350ml[mean(125±50)ml]．The number of dissected lymph nodes was 18-40[mean(26±6)]． No mortality was reported．The hospital stay was 7-12d[mean(9±2)d]. There was no recurrence during the follow-up period of 6-12 months．Conclusion TLG for gastric cancer is a safe and feasible procedure. |
