      【摘要】:目的 比较不同非甾体类抗炎滴眼液与类固醇滴眼液对白内障患者术后炎症反应的治疗效果。方法:128例在我院行白内障手术的患者术后被随机分为3组,分别接受非甾体类抗炎滴眼液溴芬酸滴眼液(溴芬酸组)、酮咯酸氨丁三醇滴眼液(酮咯酸组)及类固醇滴眼液泼尼松龙滴眼液(泼尼松龙组)的治疗。分别于术后1天、7天及15天观察检测各组患者的眼部炎症评分,眼部不良反应人数,前房水闪辉值及炎症细胞计数及房水中IL-6、TNF-a、IL-1b水平。结果:术后1天各组的炎症评分,前房水闪辉值及炎症细胞计数及房水中IL-6、TNF-a、IL-1b水平均显著高于术前(P<0.5)。术后7天和术后15天泼尼松龙组患者的炎症评分,前房水闪辉值及炎症细胞计数及房水中IL-6、TNF-a、IL-1b水平均显著低于溴芬酸组和酮咯酸组(P<0.5),而术后7天和术后15天溴芬酸组和酮咯酸组患者的这些指标之间无显著性差异。结论:本研究中的非甾体类抗炎滴眼液--溴芬酸滴眼液、酮咯酸氨丁三醇滴眼液和类固醇滴眼液--泼尼松龙滴眼液均展现较好的抗炎效果,而类固醇滴眼液--泼尼松龙滴眼液抗炎疗效显著强于两种非甾体类抗炎滴眼液。

      【Abstract】Objective: To compare the efficacy of nonsteroidal anti-inflammatory drops and steroid drop on inflammation in patients post-cataract surgery. Methods: 128 patients undergoing cataract surgery in our hospital were randomly divided into 4 groups. Patients were received treatment of nonsteroidal anti-inflammatory drops—bromfenac (bromfenac group), ketorolac (ketorolac group) and steroid drop—prednisolone (prednisolone group), respectively. The number of untoward effect patients, inflammation score, anterior chamber flare and inflammatory cell count, IL-6, TNF-a, IL-1b levels in aqueous fluid were measured on 1 day, 7 days and 15 days post-surgery. Results: The inflammation score, anterior chamber flare and inflammatory cell count, IL-6, TNF-a, IL-1b levels in aqueous fluid of the patients increased significantly compared with before surgery (P<0.05). On 7 days and 15 days post-surgery, the inflammation score, anterior chamber flare and inflammatory cell count, IL-6, TNF-a, IL-1b levels in aqueous fluid of the patients in prednisolone group were significantly lower than bromfenac group and ketorolac group, respectively (P<0.05), and there were no significantly difference of these index between bromfenac group and ketorolac group on 7 days and 15 days post-surgery. Conclusion: Both nonsteroidal anti-inflammatory drops—bromfenac, ketorolac and steroid drop—prednisolone exhibit favorable efficacy of attenuating inflammation, while the efficacy of steroid drop—prednisolone is better than nonsteroidal anti-inflammatory drops.
