      目的 探讨枸橼酸咖啡因与氨茶碱治疗早产儿原发性呼吸暂停的临床疗效、相关并发症及对早产儿血中β-EP水平的影响。方法 选择2015年1月至2017年1月在我院新生儿重症监护室病房(NICU)收治的原发性呼吸暂停的311例早产儿,随机分为枸橼酸咖啡因组(153例)、氨茶碱组(150例),比较两组治疗的有效率、并发症、不良反应及对早产儿血β-EP水平的影响等。结果 枸橼酸咖啡因组患儿用氧时间、无创通气时间、有创机械通气时间均低于氨茶碱组,差异有统计学意义(P<0.05)。枸橼酸咖啡因组与氨茶碱组比较,支气管发育不良、动脉导管未闭、早产儿视网膜病、颅内出血、高血糖、心动过速、腹胀、胃潴留、喂养不耐受的发生率均低于氨茶碱组,差异有统计学意义(P<0.05),枸橼酸咖啡因组的早产儿血β-EP水平显著低于氨茶碱组,差异有统计学意义(P<0.05)。结论 枸橼酸咖啡因治疗早产儿原发性呼吸暂停的疗效显著优于氨茶碱,能显著降低相关并发症及不良反应的发生率,降低早产儿血中β-EP的水平。

      Effects of caffeine citrate and aminophylline on apnea and effects on β-endorphin in premature infants Ji Xiaoyi,Wu Min,Yang Rongwei,Zhong Wenhua,Huang Huafei.Department of Pediatrics, Jiaxing Maternity and Child Health Care Hospital, Jiaxing, Zhejiang Province,China : Objective To investigate the efficacy and safety of aminophylline and caffeine citrate in prevention of apnea of prematurity(AOP) and its effect on β-endorphin in premature infants. Methods 303 Preterm infants admitted to NICU from Jan.2015 to Jan.2017 were randomly divided into Aminophylline group (n=150) and Caffeine citrate group (n=153).Extubation efficacy, its complications,adverse effects,and β-endorphin in two groups of patients were compared. Results The time of needing oxygen, non-invasive and invasive mechanical ventilation in the caffeine citrate group were significantly lower than those in the aminophylline group. (P<0.05)The incidence of bronchopulmonary dysplasia(BPD),patent ductus arteriosus (PDA), retinopathy of prematurity(ROP),intercranial hemorrhage(ICH),high blood glucose level,heart rate,abdominal distension,feeding intolerance in the caffeine citrate group were significantly lower than those in the aminophylline group. (P<0.05)The level of β-endorphin in the caffeine citrate group was significantly lower that in the aminophylline group.( P<0.05) Conclusion The caffeine citrate has a significant clinical effect in the treating primary apnea of preterm infants than aminophylline.It can reduce the incidence of adverse reactions and its complications,meanwhile it can ruduce the level of β-endorphin in preterm infants..
