| 目的:探讨经直肠前列腺超声提示前列腺钙化及分布情况与前列腺穿刺病理的关系。
方法：通过一项历史性队列研究，经直肠超声检查患者前列腺形态学特征、有无前列腺钙化及其分布（移行带、外周带、交界区），以前列腺钙化情况作为分组依据，比较前列腺穿刺病理结果。
结果：共有794患者纳入研究，穿刺活检提示前列腺癌患者352例，穿刺阳性率44.33%。前列腺钙化组460，前列腺无钙化组（对照组）334例，前列腺钙化组与对照组中年龄、PSA、前列腺体积等无统计学差异。前列腺钙化组中移行带存在钙化灶的患者69例,主要病理诊断是前列腺增生或前列腺炎; 外周带存在钙化灶患者50例，其中34例患者穿刺病理提示前列腺癌P<0.001。
结论：前列腺钙化在良性前列腺病变及前列腺癌患者中无明显差异，然而外周带前列腺钙化灶提示患者前列腺癌可能。 |

| OBJECTIVE: To evaluated the prevalence of Prostatic Calcification(PC)during transrectal ultrasound (TRUS) and correlate the ultrasonographic patterns with histological findings
METHOD: A prospective study of men undergoing TRUS and prostatic biopsy was designed; Representative images of the calcification in the sagittal and transverse planes were captured. PC were classified according to zone distribution into the gland: transitional zone, interface zone, and peripheral zone. TRUS findings were correlated with histologic data.
RESULT: A total of 352 patients (44.33%) had prostate cancer, 203 patients (25.57%) inflammation, and 239 patients (30.10%) had benign pathology. Interface calcification was present in 42.9% of patients. Peripheral or transitional zone calcification was unusual (6.29% and 8.69%, respectively). Of the peripheral zone calcification group patients, 68% had cancer on histology examination (P<0 .05).CONCLUSION: the prevalence of PC in peripheral zone appears to be strongly associated with prostate cancer; but was not associated with higher Gleason grade among these patients. |
