| 目的 评价老年病人经外周静脉穿刺中心静脉置管术(PICC)的方法和超声引导应用价值。 方法 回顾分析PICC置管术137例,其中73例为传统穿刺置管,64例为超声引导穿刺,对这两种方法进行对比研究,统计两种方法的一次成功率、并发症发生率及期前拔管率。 结果 传统组和超声组的一次成功率分别为71.2%和95.3%,存在显著差异(p<0.05)；传统组和超声组的并发症发生率分别为19.2%和3.1%,存在显著差异(p<0.05)；传统组和超声组的期前拔管率分别为24.7%和10.9%,存在显著差异(p<0.05)；并根据术前超声检查测量目标血管的内径进行ROC曲线分析,传统组和超声组的曲线下面积分别为0.821和0.973,存在显著差异(p<0.05)。 结论 超声实时引导PICC置管有传统穿刺法无可比拟的优越性,特别是在老年病人中值得临床推广应用。 |

| ABSTRACT Object To evaluate the application of PICC catheterization guided by ultrasound in the geriatric patients. Methods 137 persons were studied. 64 cases were guided by ultrasound while 64 cases were done by traditional method. The success rate and complication rate as well as early extubation rate of two methods were compared. The ROC curve of the target vessel was analyzed according to the diameter measured by preoperative exam. Results Success rate of ultrasound guide group and traditional group were respectively 95.3% and 71.2% (p<0.05), complication rate of two groups were respectively 3.1% and 19.2% (p<0.05), and early extubation rate of two groups were respectively 10.9% and 24.7% (p<0.05). The area under the ROC curve of two groups was 0.973 and 0.821, respectively, with significant difference (p<0.05). Conclusion PICC catheterization guided by ultrasound had unparalleled superiority to the traditional method in the geriatric patients. |
