| 目的 探讨输尿管软镜下钬激光碎石术治疗马蹄肾肾结石的可行性、临床疗效及应用前景。方法 回顾性分析本院2014年9月至2018年6月采用输尿管软镜下钬激光治疗12例马蹄肾肾结石患者的临床资料。肾结石直径约1.0-2.2cm，平均约（1.6±0.4）cm。术前均留置F6双J管1-2周，并行尿培养、泌尿系CT平扫检查。术后复查KUB及泌尿系CT，无结石或残石＜3mm视为碎石成功。结果 本组12例患者均一期完成手术，手术时间63-122min，平均87.1±18.5min，平均住院时间3.5±0.4天，术中、术后无严重并发症发生。输尿管软镜碎石成功率83.3%（10/12），2例因肾下盏结石，肾下盏肾盂夹角过小，输尿管软镜可见结石但激光光纤无法触及，术中改微通道经皮肾镜处理后碎石成功。结论 输尿管软镜下钬激光碎石术治疗马蹄肾肾结石是安全可行的，并且创伤小、并发症少，对肾中上盏结石临床疗效确切，对肾下盏结石需个体化选择。 |

| Objective To evaluate the feasibility，clinical efficacy and application prospect of the treatment of renal calculi in the horseshoe kidney lithotripsy by flexible ureteroscope with Holmium laser. Methods The clinical data of 12 patients who underwent flexible ureterscope with Holmium laser to treat renal calculi in the horseshoe kidney during Sep.2014 and Jun.2018 were retrospectively analyzed. The renal calculi size was 1.0cm-2.2cm（mean 1.6cm）.All patients were placed F6 Double-J stent and were underwent urine culture ,computed tomography of urinary before operations. After operations, the follow-up examination was performed with KUB and CT. Stone-free or residual fragments＜3mm were considered success. Results All 12 cases were successfully completed with no severe complication in the first stage. The operation time was 63-122min（mean 87.1±18.5min）.The patients were treated in the hospital mean 3.5±0.4days.The stone free rate was 83.3%(10/12). 2 cases with the small lower pole infundibulopelvic angle were successfully received microchannel percutaneous nephrolithotomy to treat the lower renal calyx calculi. Conclusion The flexible ureterscope with Holmium laser is safe，feasible，minimal invasive and effective in the treatment of upper and middle renal calyx calculi in the horseshoe kidney.It need a individualized choice for the lower renal calyx calculi in the horseshoe kidney. |
