      目的:通过观察推拿手法对家兔失神经支配骨骼肌干预后肌湿重、成肌调节因子中生肌因子5(Myogenic factor 5,Myf-5)、肌细胞生成素(Myogenin)表达的变化,探讨推拿手法对失神经支配骨骼肌的影响。方法:新西兰家兔随机分为4大组(n=30):正常对照组、模型对照组、神经生长因子(Nerve growth factor,NGF)治疗组、手法治疗组,每大组再随机分为5个亚组(n=6):2w组、3w组、1m组、2m组、4m组。分别干预后处死取材,称量肌湿重,计算术侧与健侧的肌湿重比值;检测Myf-5信使核糖核酸(mRNA)、Myogenin mRNA表达水平。结果:①腓肠肌肌湿重比:NGF治疗组在2w(0.79±0.03)、3w(0.70±0.06)、1m(0.58±0.04)、2m(0.44±0.05),手法治疗组在2w(0.72±0.05)、3w(0.55±0.04)、1m(0.55±0.09)、2m(0.49±0.03)、4m(0.50±0.03)高于模型观察组2w(0.53±0.01)、3w(0.41±0.03)、1m(0.40±0.03)、2m(0.38±0.05)、4m(0.38±0.05),差异有统计学意义(P<0.05)。手法治疗组在2w(0.72±0.05)、3w(0.55±0.04)低于NGF治疗组2w(0.79±0.03)、3w(0.70±0.06),4m(0.50±0.03)时则高于NGF治疗组(0.42±0.02),差异有统计学意义(P<0.05)。②腓肠肌Myf-5 mRNA表达:NGF治疗组在2w(18.50±1.79)、3w(12.52±0.58)、1m(5.30±1.76)、2m(2.82±0.23),手法治疗组在2w(20.48±2.20)低于模型观察组2w(40.91±10.31)、3w(25.25±1.27)、1m(13.91±1.40)、2m(8.83±0.86),手法治疗组在1m(16.50±1.17)、2m(12.20±0.84)、4m(5.32±0.60)高于模型观察组1m(13.91±1.40)、2m(8.83±0.86)、4m(3.82±0.54),差异有统计学意义(P<0.05)。手法治疗组在3w(37.21±9.10)、1m(16.50±1.17)、2m(12.20±0.84)、4m(5.32±0.60)时高于NGF治疗组3w(12.52±0.58)、1m(5.30±1.76)、2m(2.82±0.23)、4m(3.88±0.57),差异有统计学意义(P<0.05)。③腓肠肌Myogenin mRNA表达:NGF治疗组在2w(94.31±23.87)、3w(165.95±29.58)、1m(156.59±64.76)、4m(4.05±0.51),手法治疗组在4m(5.74±1.35)低于模型观察组2w(185.92±48.71)、3w(359.21±116.88)、1m(597.50±146.66)、4m(63.13±12.24),手法治疗组在2m(1026.75±132.63)高于模型观察组(168.84±31.85),差异有统计学意义(P<0.05)。手法治疗组在2w(209.80±91.25)、3w(482.37±97.95)、1m(667.73±97.95)、2m(1026.75±132.63)均高于NGF治疗组2w(94.31±23.87)、3w(165.95±29.58)、1m(156.59±64.76)、2m(140.63±40.18),差异有统计学意义(P<0.05)。结论:推拿手法可能通过改善成肌调节因子Myf-5、Myogenin表达来延缓失神经支配后骨骼肌的萎缩,远期疗效较NGF良好。

      Objective: To observe the action effects and ways of tuina therapy in delaying muscle atrophy after peripheral nerve injury. Method: Male rabbits were randomly divided into sham-operated group, model group, NGF group, tuina group. Then each group were randomly divided into 5 small groups:2 weeks, 3 weeks, 1 month, 2 months, 4 months. The ratio of gastrocnemius weight was decided by contralateral and ipsilateral gastrocnemiuses, mRNA level of Myf-5 and Myogenin were measured by Real-time Polymerase Chain Reaction (qRT-PCR) after 2 weeks, 3 weeks, 1 month, 2 months, 4 months after treatment. Results:①gastrocnemius weight ratio results: compared to model group, the ratio of gastrocnemius weight in NGF group had significantly increase at 2weeks(0.79±0.03)vs(0.53±0.01),3weeks(0.70±0.06)vs(0.41±0.03),1month(0.58±0.04)vs(0.40±0.03)and 2months(0.44±0.05)vs(0.38±0.05)points (P< 0.05);compared to model group, the ratio of gastrocnemius weight in tuina group had significantly increase at 2weeks(0.72±0.05)vs(0.53±0.01),3 weeks(0.55±0.04)vs(0.41±0.03),1month(0.55±0.09) vs(0.40±0.03),2months(0.49±0.03)vs(0.38±0.05) and 4months(0.50±0.03)vs(0.38±0.05)points (P<0.05); compared to NGF group, the ratio of gastrocnemius weight in tuina group was significantly decreased at 2weeks(0.72±0.05)vs(0.79±0.03),3weeks(0.55±0.04)vs(0.70±0.06)(P < 0.05),the ratio of gastrocnemius weight in tuina group had significantly increase at 4months(0.50±0.03)vs(0.42±0.02) (P<0.05).②mRNA level of Myf-5: compared to model group, the mRNA level of Myf-5 in NGF group had significantly decrease at 2weeks(18.50±1.79)vs(40.91±10.31),3 weeks(12.52±0.58)vs(25.25±1.27),1month(5.30±1.76)vs(13.91±1.40) and 2months(2.82±0.23)vs(8.83±0.86) points (P<0.05); compared to model group, the mRNA level of Myf-5 in tuina group had significantly decrease at 2weeks(20.48±2.20)vs(40.91±10.31),but increased at 1month(16.50±1.17)vs(13.91±1.40),2months(12.20±0.84)vs(8.83±0.86)and 4months (5.32±0.60)vs(3.82±0.54)points (P<0.05); compared to NGF group, the mRNA level of Myf-5 in tuina group was significantly increased at 3 weeks(37.21±9.10)vs (12.52±0.58),1month(16.50±1.17)vs(5.30±1.76),2months(12.20±0.84)vs(2.82±0.23) and 4 months(5.32±0.60)vs(3.88±0.57) points (P<0.05).③mRNA level of Myogenin: compared to model group, the mRNA level of Myogenin in NGF group had significantly decrease at 2weeks(94.31±23.87)vs(185.92±48.71),3weeks(165.95±29.58)vs(359.21±116.88),1month(156.59±64.76)vs(597.50±146.66)and 4months(4.05±0.51)vs (63.13±12.24)points (P<0.05);compared to model group, the mRNA level of Myogenin in tuina group was significantly increased at 2months(1026.75±132.63)vs (168.84±31.85),but decreased at 4months(5.74±1.35)vs(63.13±12.24) points (P<0.05);compared to NGF group, the mRNA level of Myogenin in tuina group had significantly increase at 2weeks(209.80±91.25)vs (94.31±23.87),3weeks(482.37±97.95)vs(165.95±29.58),1month(667.73±97.95)vs(156.59±64.76) and 2months(140.63±40.18)vs(1026.75±132.63) points(P<0.05).Conclusions: Tuina manipulation may delay skeletal muscle atrophy and denaturation, promote activating mRNA level of Myf-5 and Myogenin after peripheral nerve injury, the long-term effect is better than the treatment of NGF.
