      目的:研究尿中足细胞数量与足细胞标志蛋白水平与狼疮性肾炎组织学的关系。方法:选择2017年10月-2019年9月我院收治的71例系统性红斑狼疮患者并分为肾炎组41例及非肾炎组30例,检测并比较患者尿中足细胞数量和足细胞标志蛋白水平。结果 肾炎组CH50水平显著低于非肾炎组,而血肌酐和24 h尿蛋白量显著高于非肾炎组,差异有统计学意义(均P<0.05);肾炎组尿中足细胞数量和足细胞标志蛋白水平均显著高于非肾炎组(P<0.05);混合型尿中足细胞数量显著高于Ⅲ、Ⅳ、Ⅴ型(P<0.05),Ⅳ型尿中足细胞数量显著高于Ⅲ、Ⅴ型(P<0.05);肾炎组不同组织分型患者尿中足细胞标志蛋白水平比较差异无统计学意义(P>0.05);狼疮性肾炎患者尿中足细胞数量与足细胞标志蛋白水平呈显著正相关关系(r =0.563,P<0.05),狼疮性肾炎患者尿中足细胞数量与细胞性新月体、肾小球增殖和肾间质纤维化呈显著正相关关系(r=0.514、0.457、0.385;P=0.008、0.012、0.033);尿中足细胞标志蛋白与中性粒细胞浸润呈显著正相关关系(r=0.359,P=0.041),与纤维性新月体、肾小球硬化呈显著负相关关系(r=-0.413、-0.394;P=0.024、0.030)。结论 狼疮性肾炎患者尿中足细胞数量和足细胞标志蛋白水平均明显升高,与肾炎组织学分类有关,可评估疾病严重程度。

      Objective: To study the relationships between the number of podocyte and the level of podocyte marker protein with lupus nephritis histology. Methods: 71 patients with systemic lupus erythematosus admitted to our Hospital from October 2017 to September 2019 were divided into nephritis group (41 cases) and non-nephritis group (30 cases). The number of podocytes in urine and the level of podocyte marker protein were detected and compared. Results: The CH50 level of nephritis group was significantly lower than that of non nephritis group, while the serum creatinine and 24 h urine protein were significantly higher than that of non nephritis group (P < 0.05); The number of podocyte and the level of podocyte marker protein in nephritis group were significantly higher than those in non nephritis group (P<0.05); the number of podocytes in mixed urine was significantly higher than that in type Ⅲ, Ⅳ and Ⅴ (P<0.05), the number of podocytes in type Ⅳ urine was significantly higher than that in type Ⅲ and Ⅴ (P<0.05); there was no significant difference in the level of urinary podocyte marker protein in different tissue types of nephritis group (P > 0.05); There was a significant positive correlation between the number of podocytes in urine and the level of podocyte marker protein in patients with lupus nephritis (r = 0.563, P < 0.05).the number of urinary podocytes in the patients with lupus nephritis was significantly positively correlated with the crescent body, glomerular proliferation and renal interstitial fibrosis (r=0.514, 0.457, 0.385; P=0.008, 0.012, 0.033), there was a significant positive correlation between urinary podocyte marker protein and neutrophil infiltration (r=0.359, P=0.041), was significant negative correlations with fibrous crescentic and glomerular sclerosis (r=-0.413, -0.394; P=0.024, 0.030). Conclusion The number of podocyte and the level of podocyte marker protein in patients with lupus nephritis are increased significantly, it is related to the histologic classification of nephritis and can be used to assess the severity of the disease.
