| 目的 研究TNF-α、IL-4在肛周脓肿和肛瘘组织中的表达及意义。方法 回顾性分析120例首发肛周脓肿或肛瘘的患者资料,应用免疫组织化学染色法对脓肿壁和瘘管病理标本中TNF-α、IL-4的表达进行检测,并分析TNF-α、IL-4表达的相关性。结果 TNF-α的表达阳性率在肛周脓肿组、肛瘘病程＜3月组、肛瘘病程3～6月组、肛瘘病程＞6月组分别为80%、90%、83%、73%,阳性率比较差异有统计学意义(P＜0.05)；各组中IL-4的表达阳性率分别为70%、83%、63%、50%,阳性率比较差异有统计学意义(P＜0.05),且TNF-α、IL-4的表达呈正相关(rs=0.707,P<0.05)。结论 TNF-α和IL-4在肛周脓肿、肛瘘炎症反应中具有较高的阳性率表达,且TNF-α和IL-4在不同病程时期的表达呈正相关,提示TNF-α、IL-4参与肛周脓肿及肛瘘的发生及发展。 |

| ABSTRACT Objective To investigate the significance and expression of TNF-α and IL-4 in the perianal abscess and anal fistula. Methods Clinical data of 120 patients with first-episode perianal abscess or anal fistula were analyzed retrospectively. The expression of TNF-α and IL-4 were detected by Immunohistochemical staining on the pathological specimens of abscess wall and fistula, and the correlation between TNF-α and IL-4 expression was analyzed. Results In the perianal abscess group, anal fistula course＜3 months group, anal fistula course 3～6 months group, and anal fistula course＞6 months group, the positive rates of TNF-α expression were 80%, 90%, 83%, 73% respectively. The difference of expression rate was statistically significant (P＜0.05); The positive rate of IL-4 expression in each group was 70%, 83%, 63% and 50%, respectively, The difference of expression rate was statistically significant (P＜0.05), and the expressions of TNF-α and IL-4 were positively correlated(rs=0.707，P＜0.05). Conclusion TNF-α and IL-4 had higher positive expression rates in the inflammatory response of perianal abscesses and anal fistula. Moreover, the expression of TNF-α and IL-4 in different disease stages was positively correlated , suggesting that TNF-α and IL-4 are involved in the occurrence and development of perianal abscess and anal fistula. |
