      目的:研究类风湿关节炎(RA)外周血中调节性B细胞(Breg)、髓系树突状细胞(mDC)和辅助性T细胞(Th细胞)水平与其病情程度及疾病活动程度的相关性。方法:选取我院于2017年8月至2019年12月收治的96例RA患者,根据关节功能状态分为I级、II级、III级、IV级,根据疾病活动程度分为高度活动度、中度活动度、低度活动度,同时选取同期与我院体检健康者为对照组(60例)。检测外周血中Breg、mDC和Th细胞水平,分析各组间细胞水平差异,采用Pearson相关性分析各细胞水平与疾病活动程度的相关性。结果:研究组患者Breg水平显著低于对照组,而mDC、Th1、Th17显著高于对照组(P<0.05)。三组患者外周血中Breg、mDC和Th细胞水平差异显著,其中低度活动组患者Breg水平显著高于中度、高度活动组患者,而mDC、Th1、Th17水平显著低于中度、高度活动组患者(P<0.05)。四组患者外周血中Breg、mDC和Th细胞水平差异显著,其中Ⅰ级Breg水平显著高于Ⅱ级、Ⅲ级、Ⅳ级,而mDC、Th1、Th17水平显著低于Ⅱ级、Ⅲ级、Ⅳ级(P<0.05)。Pearson相关性分析结果显示Breg与DAS28评分呈负相关(r=-0.721;P<0.001),mDC、Th1、Th17与DAS28评分呈现正相关性(r=-0.570,0.724,0.797;P<0.001)。结论:RA患者外周血中Breg、mDC和Th细胞水平均异常表达,其中Breg细胞水平与活动度严重程度负相关,mDC和Th细胞水平与活动度严重程度正相关。

      objective: To study on the relationship between the level of Breg, mDC and Th cells in the peripheral blood of RA patients and their disease severity and disease activity. Methods: 96 RA patients who were admitted to our hospital from August 2017 to December 2019 were divided into grade I, grade II, grade III and grade IV according to the joint function state. According to the disease activity degree, they were divided into severe activity degree, moderate activity degree and low activity degree. At the same time, 60 healthy persons who were examined in our hospital and in the same period were selected as the control group. The levels of Breg, MDC and Th cells in peripheral blood were measured, and the differences of cell levels among groups were analyzed. The correlation between cell levels and disease activity was analyzed by Pearson correlation. Results: The Breg level of patients in the study group was significantly lower than that in the control group, while the mDC, Th1 and Th17 were significantly higher than that in the control group (P<0.05). There were significant differences in peripheral blood Breg , mDC and Th cell levels among the three groups. The level of Breg in the low activity group was significantly higher than that in the moderate and high activity group, while the levels of mDC, Th1 and Th17 were significantly lower than that in the moderate and high activity group (P < 0.05). The levels of peripheral blood Breg, MDC and Th cells in the four groups were significantly different, among which the level of Breg in grade I was significantly higher than that of grade II, III and IV, while the levels of MDC, Th1 and Th17 were significantly lower than that of grade II, III and IV (P < 0.05). Pearson correlation analysis showed that Breg was negatively correlated with DAS28 score (r = -0.721; P < 0.001), MDC, Th1 and Th17 were positively correlated with DAS28 score (r = -0.570, 0.724, 0.797; P < 0.001). Conclusion: The expression of Breg, MDC and Th cells in the peripheral blood of RA patients is abnormal. The level of Breg cells is negatively correlated with the severity of activity, and the level of MDC and Th cells is positively correlated with the severity of activity.
