| 摘 要:目的 了解杭州地区儿童临床病原菌流行病学分布情况,为儿童临床疾病的检验及诊疗提供科学依据。方法 收集杭州市儿童医院2016年01月至2019年12月所有就诊患儿临床病原菌检测标本,统计并分析不同种类细菌检出率、不同类型临床标本阳性率、及主要细菌检出率构成比,并对2016-2019年主要检出菌的常用抗生素耐药情况简单分析。结果 2016-2019年不同类型细菌检出率主要以G+、G- 菌为主;2016-2019年痰标本、咽拭子、脓液等标本阳性率相对稳定,尿液标本2018-2019较2016-2017增高,具有统计学差异;2016-2019年G+检出菌以金黄色葡萄球菌、A群化脓性链球菌、肺炎链球菌等为主、G- 菌主要以大肠埃希菌、沙门菌属、流感嗜血杆菌等为主;2016-2019年,金黄色葡萄球菌及A群化脓性链球菌、流感嗜血杆菌对主要抗菌药物耐药性变化有所不同。 结论 积极开展临床微生物检验能够实时监测临床疾病感染,通过分析其流行病学状态,不仅提升临床儿童疾病的诊疗也利于院感的控制。 |

| Abstract: Objective To investigate the epidemiological distribution of clinical pathogenic bacteria in Hangzhou, and provide scientific basis for the examination and diagnosis of clinical diseases in children. Methods Collect the clinical pathogen detection specimens of all children in the Hangzhou Children's Hospital from January 2016 to December 2019, and analyze the detection rate of different types of bacteria, the positive rate of different types of clinical specimens, and the composition ratio of the main bacteria. Then analysis the common antibiotic resistance of the main detected bacteria. Results The detection rate of different types of bacteria is mainly G+ (S. aureus, GAS, SP, etc. ) and G- bacteria ( E. coli, Salmonella, Haemophilus influenzae, etc. ) in 2016-2019. The positive rates of sputum specimens, throat swabs and pus were relatively stable, however urine specimens increased from 2018 to 2019 compared with 2016 to 2017, with statistical differences. From 2016 to 2019, the changes of the drug resistance of S. aureus, GAS, and Haemophilus influenzae were different. Conclusion Perform clinical microbiological testing can monitor clinical disease infections in real time. Analyzing epidemiological status plays a role in the diagnosis and treatment of clinical children diseases and the control of hospital infections. |
