| 目的:探讨强制性使用对正中神经损伤后功能恢复的临床疗效。
方法:选取42例正中神经损伤患者根据数字随机表法分为对照组(n=21)与实验组(n=21),对照组采用常规康复治疗手段,实验组在常规康复治疗手段基础上增加强制性使用疗法(CIMT),分别在治疗前及治疗8周后评估指浅屈肌、拇对掌肌及第1、2蚓状肌肌力,采用表面肌电分析系统SA7550测量正中神经传导速度与波幅,在治疗8周后评估患肢正中神经功能优良率。
结果:治疗8周后指浅屈肌、拇对掌肌及第1、2蚓状肌肌力、正中神经传导速度及波幅较治疗前明显改善,差异具有显著性意义(P<0.05),且实验组明显优于对照组(P<0.05)。治疗8周后正在神经功能评定优良率实验组较对照组明显优化(P<0.05).
结论:强制性使用对正中神经损伤后功能修复效果良好,可有效改善正中神经功能、残存肌力、神经传导速度及波幅,可作为临床治疗的一种辅助手法。

| Objective: To investigate the clinical effect of Constrained-induced movement therapy on functional recovery after median nerve injury
Methods: 42 patients with median nerve injury were selected according to table Numbers randomly divided into control group (n = 21) and experimental group (n = 21), control group using conventional rehabilitation treatments, the experimental group on the basis of conventional rehabilitation treatments increased Constrained-induced movement therapy (CIMT), respectively before treatment and 8 weeks after treatment with musculus flexor digitorum superficialis strength assessment, opponens pollicis, the first and second lumbrical muscles strength, adopts the methods of electric SA7550 median nerve conduction velocity and wave amplitude measurement analysis system, in 8 weeks after treatment to assess the rate of good limb median nerve function.
Results: After 8 weeks of treatment, the muscle strength, conduction velocity and amplitude of the ulnar nerve of the musculus flexor digitorum superficialis, opponens pollicis, the first and second lumbrical muscles strength were significantly improved (P < 0.05), and the experimental group was significantly better than the control group (P < 0.05).After 8 weeks of treatment, the excellent and good rate of median nerve function assessment in the experimental group was significantly better than that in the control group (P < 0.05).
Conclusion: Constrained-induced movement therapy can effectively improve median nerve function, residual muscle strength, nerve conduction velocity and amplitude after median nerve injury. It can be used as an auxiliary technique in clinical treatment.
