| 摘要：目的 研究派能达胶囊联合半量雄激素+环孢素治疗慢性再生障碍性贫血（chronic aplastic anemia，CAA）的临床疗效，并探索性建立女性患者雄激素治疗后的雄性化评估量表。方法 纳入2015年7月至2020年5月在5家综合性医院血液科诊治的女性慢性再生障碍性贫血患者，随机分至观察组、对照组；其中观察组予以派能达胶囊（320mg/d）联合半量雄激素（安特尔80mg/d）+环孢素治疗，对照组予以足量雄激素（安特尔160mg/d）+环孢素治疗；4周为1疗程，总共6疗程，治疗后进行两组患者的疗效评价以及雄性化症状评分。结果 1.观察组西医疗效评价总有效率84.6%，对照组总有效率81.0%，两组间外周血象三系均无统计学差异。2.观察组雄性化评价平均得分为3.4±1.9/例患者，明显低于对照组的7.1±2.3/例，差异显著(P＜0.01)，提示观察组雄性化程度明显降低。3.观察组中医症候积分总有效率为92.3%，明显高于对照组的66.7%，观察组疗效明显优于对照组(P＜0.05)。4.观察组治疗后生活质量评分（汉化版SF-36量表）多维度高于对照组（P＜0.05）。结论 派能达胶囊可以代替半量雄激素治疗慢性再障而维持外周血象，能够明显减轻女性患者雄激素所致的雄性化副作用，并能有效地改善中医证候及提高生活质量。 |

| Abstract:Objective It studed the clinical efficacy of Pai-Neng-Da capsule combined with half amount of andriol and cyclosporine in the treatment of chronic aplastic anemia (CAA), and explore the establishment of masculinization scale for female patients after andriol treatment.Methods Female patients with chronic aplastic anemia treated in hematology departments of 5 general hospitals from July 2015 to May 2020 were enrolled and randomly divided into observation group and control group. The observation group was treated with Pai-Neng-Da capsule (320mg/d) combined with half amount of andriol (Antel 80mg/d) + cyclosporine, while the control group was treated with full amount of andriol (Antel 160mg/d) + cyclosporine. Four weeks was one course of treatment, and a total of 6 courses of treatment were performed. After treatment, efficacy evaluation and masculinization symptom score were conducted for the two groups of patients.Results 1. The total effective rate was 84.6% in the observation group and 81.0% in the control group, and there was no statistical difference between the two groups in the three series of peripheral blood images. 2. The average score of masculinization evaluation in the observation group was 3.4±1.9/ case, which was significantly lower than that in the control group (7.1±2.3/ case, P < 0.01), indicating that the degree of masculinization in the observation group was significantly reduced. 3. The total effective rate of TCM syndrome score in the observation group was 92.3%, which was significantly higher than that in the control group (66.7%), and the curative effect in the observation group was significantly better than that in the control group (P < 0.05). 4. The multidimensional quality of life score (Chinese version of SF-36 scale) in the observation group was higher than that in the control group (P < 0.05).Conclusion Pai-Neng-Da capsule can replace half amount of andriol in the treatment of chronic aplastic anemia and maintain peripheral blood image, which can significantly reduce the side effect of andriol caused by masculinization in female patients, and effectively improve the TCM syndrome and improve the quality of life. |
