| 【摘要】 目的: 探讨年轻乳腺癌的超声多模态的表现。方法: 探讨211个手术病理证实的年轻乳腺癌病灶,全部病例都采取二维超声、彩色多普勒、自动乳腺全容积成像(ABVS)与弹性成像。分析病灶的形态、方位、边缘、内部回声、是否存在钙化、血流、冠状面情况、周围有无声晕与弹性评分状况,且结合乳腺影像报告和数据系统(BI-RADS)予以评分。结果: 211个年轻乳腺癌病灶,导管原位癌44个(20.85%),浸润性导管癌167个(79.15%),其中浸润性特殊性粘液癌有7个。分子分型中的Luminal A型110个(52.13%),B型44个(20.85%),阳性型18个(0.47%),三阴性型39个(18.48%)。超声多模态影像表现:肿块型151个(71.56%),非肿块型60个(28.44%)。其中肿块型中形状不规则137个(90.73%),规则及浅分叶状14个(9.27%),内部呈低回声137个(90.73%),呈混合回声14个(9.27%);非肿块中腺体局部回声减低42个(70%),肿块型及非肿块型中表现为冠状面结构扭曲、血流高阻及腋窝淋巴结转移特征的比率均不高,范围10.0%-39.7%,而出现富血供及高弹性评分的比例较高,范围63.6%-72.2%,肿块型及非肿块型乳腺癌病灶在冠状面结构扭曲、彩色血流、阻力指数、弹性评分与腋窝淋巴结转移特征间均无统计学意义(P>0.05),而病灶中伴微钙化有统计学意义(P=0.000)。术前BI-RADS分类诊断为4B-5类145个,4A类60个,0-3类6个,诊断符合率97.16%。 |

| 【Abstract】 Objective To investigate the multi-modal ultrasound manifestations of young breast cancer. Methods A total of 211 young patients with breast cancer were analyzed.All cases were detected by two-dimensional ultrasound,color Doppler, automatic breast volume imaging (ABVS) and elastography. The shape, location, edge, internal echo nature, calcification, blood flow, coronal plane, adjacent changes and elasticity score of the lesions were analyzed and scored according to the breast imaging report and data system (BI-RADS).Results In 211 cases of young breast cancer, 44 cases (20.85%) were ductal carcinoma in situ, 167 cases (79.15%) were invasive ductal carcinoma.Among the molecular typing, Luminal A was found in 110 cases (52.13%), Luminal B in 44 cases (20.85%), HER2 positive in 18 cases (0.47%), triple negative in 39 cases (18.48%).There were 151 cases (71.56%) of mass type, 60 cases (28.44%) of non mass type, 137 cases (90.73%) of irregular shape, 14 cases (9.27%) of regular and shallow lobulated shape, 137 cases (90.73%) of internal hypoechoic, 14 cases (9.27%) of mixed echo, 42 cases (70%) of local echo of gland in non mass,There was no significant difference in the incidence of distortion of coronal structure, high resistance and axillary lymph node metastasis in both mass and non mass breast cancer lesions, ranging from 10.0% to 39.7%, while the proportion of high blood supply and hard elasticity score was high, ranging from 63.6% to 72.2%,The analysis showed that there were no significant differences in coronal structure distortion, color blood flow, resistance index, elasticity score and axillary lymph node metastasis had no statistical value (P> 0.05), but calcification had statistical value ( P= 0.000).According to BI-RADS classification, 145 cases were classified as 4b-5, 60 cases as 4a, 6 cases as 0-3. The diagnostic coincidence rate was 97.16%.Conclusions Young breast cancer is also characterized by infiltrating ductal carcinoma and Luminal A type in pathology. Irregular hypoechoic mass type is the obvious sign on ultrasonic image. The high proportion of rich blood supply and hard elasticity score can help the diagnosis of breast cancer. Non mass breast cancer with high calcification is the feature of the lesion. |
