| 目的:探讨高血压患者血清胱抑素C(CysC)与踝臂指数(ABI)水平,及其与早期肾损害程度的相关性以及预测价值。方法:选择124例本院治疗的高血压患者,根据是否存在肾损害分为:损伤组(60例,存在肾功能损伤)和单纯组(64例,仅有高血压但肾功能正常)。选取同时期60例同期健康体检者为对照组。检测比较三组血清CysC、β2微球蛋白(β2-MG)、尿微量白蛋白(mAlb)、血肌酐(Scr)及尿素氮(BUN)差异;检测比较三组ABI指标差异;应用Pearson直线相关分析评估血清CysC及ABI分别与mAlb的相关程度。应用多元Logistic回归分析高血压患者肾功能损伤的危险因素。绘制受试者工作特征曲线(ROC)评估血清CysC及ABI对高血压患者肾功能损伤的早期诊断价值。结果:损伤组血清CysC、β2-MG、mAlb水平高于单纯组(均P<0.05),且单纯组高于对照组(均P<0.05);而三组血清Scr及BUN水平无显著差异(均P>0.05)。损伤组ABI高于单纯组(均P<0.05),且单纯组高于对照组(均P<0.05)。血清CysC与mAlb呈正相关(r=0.821,P=0.027),而ABI与mAlb呈负相关(r=-0.853,P=0.023)。多元Logistic回归分析显示,血清CysC为EH患者肾功能损伤的危险因素(OR=2.872, P=0.020),而ABI为保护性因素(OR=0.624, P=0.032)。ROC曲线显示,血清CysC及ABI截断值分别为1.75mg/L及0.85,两者联合诊断高血压患者肾功能损伤的敏感度为89.9%,特异度为87.0%。结论:血清CysC联合ABI早期诊断高血压患者肾功能损伤的准确度较好。 |

| Objective To evaluate the levels of serum cystatin C (CysC) and ankle brachial index (ABI) in patients with essential hypertension, and to analyze the clinical value of the CysC and ABI to the early renal impairment. Methods The 124 patients with hypertension in our hospital were collected and divided into two groups according to the renal impairment, which were impairment group (60 cases, hypertension with the renal impairment) and simple group (60 cases, hypertension without the renal impairment). The contemporaneous 60 healthy examinees were selected as control group. The serum levels of CysC, β2-micoglobulin(β2-MG), serum creatinine (Scr), blood urea nitrogen (BUN) and the urine microalbumin (mAlb) were detected and compared among three groups. The indexes of ABI were detected and compared among three groups. The correlations of CysC and ABI to mAlb were analyzed by Pearson analysis. The risk factors of early renal impairment in patients with hypertension to the clinical indexes were confirmed by multivariate Logistic analysis. The diagnostic value of the serum levels of CysC and ABI to the early renal impairment of the patients with hypertension were analyzed by receiver operating characteristic curves (ROC). Results The levels of serum CysC, β2-MG and urine mAlb in impairment group were highest, and the simple group were the second highest, and the control group were lowest (all P<0.05). But the indexes of Scr and BUN were no different among three groups (all P>0.05). The levels of ABI in impairment group were highest, and the simple group were the second highest, and the control group were lowest (all P<0.05). The positive relationships of serum levels of CysC to the levels of mAlb were confirmed by Pearson analysis(r=0.821, P=0.027), but the negative relationships of the ABI values to the levels of mAlb were also confirmed (r=-0.853, P=0.023). Multivariate Logistic analysis showed that the serum CysC was independent risk factor to early renal impairment in patients with hypertension (OR=2.872, P=0.020), but the ABI was the protective factor (OR=0.624, P=0.032). When the cut-off point of serum CysC level was 1.75mg/L and ABI value was 0.85, the sensitivity was 89.9% and specificity was 87.0%. Conclusion The serum CysC level combined with ABI can assess the early renal impairment in patients with hypertension. |
