| [目的]总结林胜友主任中医师运用附子治疗恶性肿瘤的临床经验。[方法]通过跟师临证学习,整理归纳林师运用附子的临床病案,分析林师应用附子的思路、配伍及剂量,并举验案加以佐证。[结果]林师临证善用附子,常将附子与仙茅、仙灵脾、仙鹤草或龟鹿二仙胶配伍,促进益气养血,治疗骨髓抑制；附子与麻杏甘石汤配伍,温肾资肺,治疗放射性肺炎；附子与黄芪配伍,温阳通络,治疗手足综合征；附子与一贯煎配伍,阳中求阴；附子与补中益气汤、四君子汤类方配伍,增强健脾益气；附子与苍术、草果、槟榔等配伍,温阳化湿。[结论]林师立足辨证论治的原则,在辨病与辨证相结合的基础上,把握恶性肿瘤综合治疗特点,灵活地将附子合方入药,临床思路及用药特点值得借鉴。 |

| [Objective] To summarize Chief TCM physician LIN Shengyou's clinical experience in the treatment of malignant tumor with Radix Aconite Carmichaeli. [Methods]Through following teacher in clinical work, sorting out the clinical cases, analyzing the ideas, compatibility and dosage of using Radix Aconite Carmichaeli.
Besides, a clinical case was cited as proof. [Results] Master Lin is good at using Radix Aconite Carmichaeli in clinical work. Radix Aconite Carmichaeli is often combined with Curculigo orchioides Gaerten, Epimedium, Agrimoniae or Guilu Erxian Jiao to promote qi and blood, treating myelosuppression. Radix Aconite Carmichaeli is combined with maxing shigan soup to warm kidney and regulate lung, treating radiation pneumonia. Radix Aconite Carmichaeli is combined with Astragali Radix to warm Yang and dredge meridian, treating Hand-foot syndrome. Radix Aconite Carmichaeli is combined with Yiguanjian to reinforce Yin from Yang. Radix Aconite Carmichaeli is combined with Buzhong Yiqi Decoction and Four Gentlemen Decoction and its modified prescriptions to reinforce invigorating spleen and tonifying qi. Radix Aconite Carmichaeli is combined with Rhizoma Atractylodis, Amomum tsao-ko, beltel-nut and so on to warm Yang and resolve dampness. [Conclusion] Master Lin takes syndrome differentiation and treatment as the principle, basing on the combination of disease differentiation and syndrome differentiation, grasping the characteristics of comprehensive treatment of malignant tumor, using Radix Aconite Carmichaeli flexibly, whose clinical thinking and medication characteristics are worth learning. |
