| 目的 分析乳腺粘液癌的超声声像图特征, 探讨其超声诊断价值。方法 回顾性分析经病理证实的80例乳腺粘液癌超声征象,包括病灶位置、形态、内部回声,内部有无钙化、后方回声有无增强、血流信号、血流阻力情况、弹性评分以及淋巴结是否有转移。了解超声对乳腺粘液癌诊断的符合率。结果 乳腺粘液癌超声特征分析显示病灶以位置腺体边缘(60%)、突入脂肪层(58%)及外形呈规则或浅分叶(58%)为主,但其间差异均无统计学意义(P>0.05),且在彩色血流、阻力指数及弹性评分间亦均无统计学意义(P>0.05),但在内部回声(呈不均质中等、偏高回声75%;无钙化80%)、后方回声增强(90%)及腋窝无淋巴结转移(95%)间差异均有统计学意义(P<0.05)。乳腺粘液癌术前BI-RADS分类为4A-5类74例,3类6例,以BI-RADS4A类及以上者诊断为恶性,诊断符合率92.5%。结论: 乳腺粘液癌的超声声像图有一定的特征性,以中等及偏强回声、不伴钙化、后方回声增强及无腋窝淋巴结转移为明显征象,通过对超声声像图的认识,有助于提高其诊断率,降低误诊及漏诊。 |

| Objective To analyze the Ultrasonography characteristics of mucinous breast carcinoma (MBC) and explore its diagnostic value. Methods The ultrasonography characteristics of 80 cases of MBC confirmed by pathology were analyzed retrospectively, including lesions location, morphology, internal echo, internal calcification, posterior echo, color doppler, resistance index, elasticity score and lymph node metastasis. to investigate the diagnostic value of ultrasonography in MBC. Results MBC ultrasonography characteristics showed that lesions mainly located in edge glands (60%), into fat layer (58%) and shape the rules (58%), but there were no significant difference (P > 0.05), and between color doppler, resistance index and elasticity score also were no significant difference (P > 0.05), there were significant difference in the internal echo, posterior echo enhancement and axillary lymph node metastasis (P < 0.05). and the diagnostic coincidence rate was 92.5%. Conclusions MBC has certain Ultrasonography characteristics, with iso-echoic or Shyper-echoic, no calcification, posterior echo enhancement and no axillary lymph node metastasis as obvious characteristics. through the understanding of ultrasonography imaging, it was helpful to improve the diagnostic value of MBC. |
