| 目的:评价联合(1,3)-?-D葡聚糖(BDG)和降钙素原(PCT)检测在早期鉴别菌血症/念珠菌血症中的诊断价值。方法:收集2019年1月-2020年3月期间,血培养阳性患者的血液标本同时进行BDG和PCT检测,比较单独或联合使用BDG和PCT诊断菌血症和真菌血症的敏感性,特异性,阳性预测值(PPV),阴性预测值(NPV)。结果:以BDG>100 pg / mL,PCT≤1 ng/mL为界点诊断念珠菌血症的敏感性为66.7%,特异性为96.7%,PPV为80%, NPV为93.5%; 以BDG≤100 pg / mL,PCT>1 ng/mL为界点诊断菌血症的敏感性为66.7%,特异性为94.4%,PPV为98.4%,NPV为56.7%,联合检测的诊断率高于单一检测。结论:联合BDG和PCT检测有助于早期鉴别菌血症/念珠菌血症。 |

| Objectives: To evaluate the diagnostic value of combined (1,3) -?-D glucan (BDG) and procalcitonin (PCT) tests in the early differentiation of bacteremia/candidemia. Methods: To compare the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of BDG and PCT alone or in combination for the diagnosis of bacteremia and fungemia by collecting simultaneous BDG and PCT tests in patients with a positive first blood culture between January 2019 and March 2020. Results: The cut-off point of BDG>100 pg/mL and PCT≤1 ng/mL had a sensitivity of 66.7%, specificity of 96.7%, PPV of 80%, and NPV of 93.5% for the diagnosis of candidemia; the cut-off point of BDG≤100 pg/mL and PCT>1 ng/mL had a sensitivity of 66.7%, specificity of 94.4%, PPV of 98.4%, and NPV of 56.7% for the diagnosis of bacteremia, and the diagnostic yield of the combined test was higher than that of the single test. Conclusions: Combined BDG and PCT testing helps in early identification of bacteremia/candidemia. |
