      目的 探讨虚拟现实(virtual reality,VR)技术与上肢康复机器人训练相结合对脑卒中患者上肢功能恢复的影响。方法 60例脑卒中患者随机分为VR组、机器人组和联合组,每组20例,3组均给予常规康复治疗,VR组额外增加VR技术训练;机器人组额外增加上肢康复机器人训练;联合组增加上肢康复机器人训练和VR技术训练,各组均持续治疗4周。采用Fugl-Meyer运动评分量表上肢部分(FMA-UE)、中文版上肢动作研究量表(C-ARAT)和改良的Barthel指数(MBI)评估患者治疗前后的功能。结果 治疗4周后,3组患者FMA-UE、C-ARAT及MBI评分较治疗前均有明显改善(P<0.05),且联合组治疗后FMA-UE[(36.18±6.34)分]、C-ARAT[(30.19±5.17)分]和MBI[(68.93±9.45)分]评分显著优于VR组和机器人组,差异具有统计学意义(P<0.05)。结论 虚拟现实技术与上肢康复机器人训练相结合,对脑卒中患者的上肢功能和日常生活活动能力有显著改善作用。

      Objective To investigate the effect of combining virtual reality (VR) technology and upper limb rehabilitation robot training on the functional recovery of upper limbs in stroke patients. Methods Sixty stroke patients were randomly divided into VR group, robot group and combined group, 20 patients in each group. All three groups were given conventional rehabilitation treatment, VR group added additional VR technology training; robot group added additional upper limb rehabilitation robot training; combined group added upper limb rehabilitation robot training and VR technology training. The Fugl-Meyer Assessment Upper Extremity Scale (FMA-UE), the Chinese version of the Action Research Arm Test (C-ARAT) and the modified Barthel Index (MBI) were used to evaluate the function of patients before and after treatment. Results After 4 weeks of treatment, the FMA-UE, C-ARAT and MBI scores of patients in all three groups improved significantly compared with those before treatment (P<0.05), and the FMA-UE [(36.18±6.34) points], C-ARAT [(30.19±5.17) points] and MBI [(68.93±9.45) points] scores of the combined group were significantly better than those of the VR group and robot group, with statistically significant differences (P<0.05). Conclusion The combination of virtual reality technology and upper limb rehabilitation robot training had a significant improvement on upper limb function and activities of daily living of stroke patients.
