      目的:检测系统性红斑狼疮(Systemic Lupus Erythematosus, SLE)尿液IFN-α水平及TRP-KYN通路与临床特征的相关性,以寻求评价SLE患者临床表型的尿液生物标志物。方法:纳入就诊于浙江大学附属第二医院及浙江中医药大学附属第二医院风湿免疫科并确诊为SLE的患者,记录患者一般性别、年龄、病程等流行病学数据,以及疾病活动评分(SLEDAI)、狼疮生存质量评价(LupusQoL),并采集患者尿液,选用酶联免疫吸附法(Enzyme Linked Immunosorbent Assay,ELISA)法检测尿液中的IFN-α、色氨酸(Tryptophan,TRP)和犬尿氨酸(Kynurenine,KYN)水平,并用KYN/TRP的比值表示TRP-KYN通路上关键酶IDO酶的活性。运用描述性分析、秩和检验和Spearman相关性分析,以 P<0.05 双尾为差异具有统计学意义,分析IFN-α及TRP-KYN通路与临床特征的相关性。结果:尿液IFN-α与SLE患者的疾病活动度显著正相关(R=0.254,P<0.05),但TRP-KYN通路与SLE患者临床特征无明显相关性,且尿液中IFN-α与TRP-KYN通路也无显著相关性。结论:SLE患者尿液中IFN-α水平与SLE疾病活动度呈显著正相关,可作为疾病活动的参考指标,提示IFN-α可作为SLE尿液中的新型标志物。

      Objective: To investigate the association between urinary IFN-α level and TRP-KYN pathway with clinical characteristics in patients with systemic lupus erythematosus (SLE), and to seek urine biomarkers to evaluate clinical phenotypes in SLE patients. Methods: Patients diagnosed with SLE who visited the second affiliated hospital of Zhejiang university and the department of rheumatology and immunology of the second affiliated hospital of Zhejiang Chinese medicine university were included. Epidemiological data including gender, age, disease course, SLE disease activity score (SLEDAI) and LupusQoL were recorded, and urine was collected. Enzyme Linked Immunosorbent Assay (ELISA) was used to detect the levels of IFN-α, Tryptophan (TRP) and Kynurenine (KYN) in urine. The KYN/TRP ratio was used to represent the activity of IDO, a key enzyme in the TRP-KYN pathway. Descriptive analysis, rank sum test and Spearman correlation analysis were used to analyze the correlation between IFN-α and TRP-KYN pathway and clinical characteristics. And the difference of P<0.05 was statistically significant. Results: There was a significant positive correlation between urine IFN-α and disease activity in SLE patients (R=0.254,P < 0.05), but there was no significant correlation between TRP-KYN pathway and clinical characteristics of SLE patients, and there was no significant correlation between urine IFN-α and TRP-KYN pathway. Conclusion: There is a significant positive correlation between the level of IFN-α in the urine of SLE patients and SLE disease activity, which can be used as a reference indicator of disease activity, suggesting that IFN-α can be used as a novel marker in SLE urine.
