| 目的 分析原发性干燥综合征伴抑郁（pSS-MDD）的高凝血状态及解毒祛瘀滋阴方的治疗作用。方法 选取56例pSS-MDD患者为试验组、30例pSS患者为对照组，分析pSS-MDD患者的凝血功能（PT、APTT、TT、FIB和D-D）状态。将56例pSS-MDD患者再随机分为中西医组29例、西药组27例，中西医组采用解毒祛瘀滋阴方联合羟氯喹治疗，西药组采用羟氯喹治疗，观察期12周。评价两组治疗后凝血功能、血沉（ESR）、免疫球蛋白A、G、M（IgA、IgG、IgM）和SS病情评估指数（ESSDAI）、汉密尔顿抑郁量表（HAMD）评分的差异。结果 ①pSS-MDD患者FIB和D-D值较pSS对照组明显升高，差异均有统计学意义（P<0.05），FIB和D-D值与HAMD评分呈正相关关系。②完成12周观察的中西医组有27例，西药组有26例。12周时中西医组FIB、D-D低于西药组（t=-2.669、-2.425，P值均<0.05）；且ESR、IgG、IgM、ESSADI、HAMD较西药组明显下降（t=-2.675、-3.701、-2.228、-2.389、-2.041，P值均<0.05）。中西医组对抑郁的疗效明显优于西药组。结论 pSS-MDD患者存在血液高凝血状态，解毒祛瘀滋阴方联合羟氯喹能有效改善pSS-MDD患者的凝血状态和疾病活动度，提高临床疗效。 |

| Objective To investigate the effect of Jiedu Quyu Ziyin Recipe(JQZR) with Hydroxychloroquine on the Coagulation Status in primary Sj?gren’s Syndrome with depression.Methods We collected 56 cases in primary Sj?gren’s Syndrome(pSS) with depression as the experimental group and thirty pSS patients as the control group.The coagulation parameters were measured by automatic coagulation analyzer,including prothrombin time (PT),activated partial thromboplastin time (APTT),plasma fibrinogen
(FIB),D-dimer (D-D).The results were analyzed by Two-sample t-test.56 cases with pSS-MDD
were randomly divided into 2 groups.29 cases in the integration traditional chinese and western medicine group was treated with JQZR and hydroxychloroquine(HCQ) and 27 cases in the western medicine group was treated with HCQ only.Measurement of the level of erythrocyte sedimentation rate(ESR),immunoglobulin?A,immunoglobulin G,immunoglobulin M,ESSDAI index,HAMD scores in different groups were performed before treatment and after the 12 weeks.Results Compared with the pSS control group,the levels of FIB,D-D in patients with pSS-MDD group were significantly decreased(P<0.05)，meanwhile they were positively correlated with HAMD score of pSS patients.After 12 weeks observation, there were 27 cases in the western medicine group and 26 cases in the western medicine group.FIB,D-D in the integration traditional chinese with western medicine were significantly decreased(t=-2.669,-2.425,Ps<0.05).Meanwhile,ESR,IgG,IgM,ESSADI,HAMD were significantly decreased（t=-2.675,-3.701,-2.228,-2.389,-2.041,Ps<0.05）.Conclusion High coagulation state is common in patients with pSS-MDD,JQZR decoction with hydroxychloroquine can effectively improve the high clotting state and disease activity of pSS-MDD patients. |
