      目的 探讨新生儿脑膜败血伊丽莎白菌感染脑膜炎的临床特点及治疗策略;方法 回顾性分析该院一例脑膜败血伊丽莎白菌感染脑膜炎的足月新生儿病例,并结合文献复习,对该病的临床特点、诊治进行总结;结果 患儿,女,足月,4420g,出生13天时确诊脑膜败血伊丽莎白菌脑膜炎,经哌拉西林他唑巴坦、万古霉素、利福平静滴联合抗感染后治愈出院,随访无并发症。国内外数据库共检索到脑膜败血伊丽莎白菌感染的新生儿脑膜炎病例117例,连同本例共计118例。在可溯源的感染新生儿中,52.34%的病例在出生一周内发病,早产儿占60.87%。在可追踪临床结局的115名患儿中,52人(45.22%)死亡,幸存患儿中53.97%的病例出现严重后遗症;结论 脑膜败血伊丽莎白菌所致新生儿脑膜炎以早期发病为主,大多发生于早产儿,病死率高,后遗症风险大,目前尚无统一抗感染方案,治疗以联合用药为主,哌拉西林他唑巴坦联合万古霉素、利福平可作为方案参考。

      Objective To discuss the clinical characteristics and treatment strategies of neonatal Elizabethkingia meningoseptica (E. meningoseptica) meningitis. Methods This study reported a case of meningitis due to E. meningoseptica in a full-term newborn who was admitted to this hospital.The related literatures were retrospectively reviewed and the characteristics of clinical symptoms,diagnosis and treatment were summarized. Results A full-term neonate with birth weight of 4420g developed E. meningoseptica meningitis on age of 13-days-old. After treatment with piperacillin tazobactam, combined with vancomycin and rifampin, he was alive and no complications were observed during follow-up. 112 cases of neonatal meningitis associated with E. meningoseptica were retrieved from PubMed, while 5 Chinese cases had been reported from CNKI. A total of 118 cases (including this case) were reviewed. Among the traceable infected newborns, 52.34% of cases became ill within the first week of life, and premature infants accounted for 60.87%. In 115 recorded patients in whom the outcome of the infection could be traced, 52 patients or 45.22% were died, while 53.97% of surviving infants developed severe sequelae. Conclusion Neonatal E. meningoseptica meningitis correlates with early onset of clinical illness, and mostly occurs in premature infants, leading to extremely high risk of death and complications. The treatment has not been standardized as yet, and existing research is more inclined to use two or more antibiotics. The combination of piperacillin tazobactam, vancomycin and rifampin may be used as a reference for effective treatment of neonatal E. meningoseptica meningitis.
