| 目的：探讨自发性纵膈气肿的临床特点、诊断、治疗及预后。方法：报道在临床中碰到的一例以咽痛为首发症状的自发性纵膈气肿并复习相关国内外文献。结果：自发性纵膈气肿在临床上比较少见，主要以胸痛、呼吸困难和皮下气肿为主症，部分患者临床表现不典型，而以咽痛、发声困难或鼻音等为主要表现，诊断需借助胸部X线或胸部CT，治疗以保守治疗为主，预后良好，复发率低。结论：自发性纵膈气肿是一种较为罕见的良性自限性疾病，临床表现无特异性，临床医生需重视对患者病史的询问，仔细分析临床表现及体征，怀疑自发性纵膈气肿者，推荐行胸部CT检查以明确诊断，一旦确定为自发性纵膈气肿，则经保守治疗后大多预后良好，且很少复发。 |

| Abstract: Objective: To explore the clinical characteristics, diagnosis, treatment and prognosis of spontaneous pneumomediastinum. Methods: Report a case of spontaneous pneumomediastinum with sore throat as the first symptom in the clinic and review relevant literatures at home and abroad. Results: spontaneous pneumomediastinum is relatively rare in clinical practice. The main symptoms of spontaneous pneumomediastinum are chest pain, dyspnea, and subcutaneous emphysema. The clinical manifestation of some patients isn’t typical, such as sore throat, dysphonia and nasal sounds. The diagnosis depend on chest X-ray or chest CT.The treatment is mainly conservative treatment, the prognosis is good, and the recurrence rate is low. Conclusion: spontaneous pneumomediastinum is a relatively rare benign self-limiting disease with no specific clinical manifestations. Clinicians should pay attention to the patient's medical history, carefully analyze the clinical manifestations and signs.Chest CT examination is recommended to confirm the diagnosis when there is a suspected spontaneous pneumomediastinum patient. Most of the spontaneous pneumomediastinum patients have a good prognosis after conservative treatment and rarely relapse. |
