| 目的 探讨腮腺血管外皮细胞瘤的MRI表现,以期提高对本病的影像学认识。方法 回顾性分析5例经手术病理证实为腮腺血管外皮细胞瘤的临床、MRI影像及病理资料并复习相关文献。结果 5例腮腺血管外皮细胞瘤均单侧发病,3例位于右侧腮腺,2例位于左侧腮腺；3例形态规则呈类圆形,2例形态不规则；4例边界清楚,另外1例边界模糊；病变最大径约23- 51mm,平均34mm。MRI表现：平扫3例病变信号较均匀,其中2例T1WI呈低信号、T2WI呈较高信号,1例T1WI、T2WI均呈等信号；其余2例信号不均匀,1例伴钙化,1例伴出血坏死及囊变；1例病变周围可见增粗流空血管信号。增强扫描病变实质部分呈明显持续性强化,2例强化均匀,3例强化不均匀。结论 MRI增强扫描肿瘤实质部分呈明显均匀/不均匀持续强化是腮腺血管外皮细胞瘤较典型的影像表现,同时MRI检查能较好地显示肿瘤的范围。 |

| Objective To explore the MR imaging characteristics of the parotid hemangiopericytoma (PHC) with aim to improve the imaging diagnosis ability for PHC. Methods The clinical, MRI and pathological data of 5 cases of hemangiopericytoma of the parotid gland confirmed by surgery and pathology were retrospectively analyzed and related literatures were reviewed. Results All cases were unilateral, with 3 cases located in the right parotid gland and 2 cases located in the left parotid gland. 3 cases were circular and 2 cases were irregular. The boundary was clear in 4 cases and fuzzy in 1 case. The maximum diameter of the lesion was 23-51mm, with an average of 34mm. MRI findings: The lesion signal of 3 patients was uniform on plain scan, of which 2 patients showed low signal on T1WI and high signal on T2WI, and 1 patient showed isosignal on T1WI and T2WI. The signal of the other 2 cases was uneven, 1 case was accompanied by calcification, and 1 case was accompanied by hemorrhage, necrosis and cystic degeneration. In 1 case, the signal of thickened empty vessels was seen around the lesion. The enhancement in parenchyma was obvious and persistent, with uniform enhancement in 2 cases and uneven enhancement in 3 cases. Conclusion The tumor parenchyma showed obvious uniform / uneven continuous enhancement on enhanced MRI, which is a typical imaging manifestation of parotid hemangiopericytoma. At the same time, MRI can better show the scope of the tumor. |
