      目的 通过建立大鼠背部压力性损伤模型,研究活血生肌方外用在大鼠压力性损伤愈合过程中对创面组织内金属蛋白酶-9(MMP-9)及其组织抑制因子-1(TIMP-1)表达水平的影响,探讨活血生肌方对压力性损伤愈合的机制。方法 在SD雄性大鼠背部建立压力性损伤模型,每只大鼠背部各建立上下两个创面,选取成功造模大鼠60只,将上下创面分为治疗组和对照组。治疗组:创面常规处理,予活血生肌纱条外敷后用无菌纱布包扎,1次/天,直至实验结束。对照组:创面常规处理,予凡士林纱条外敷后用灭菌纱布包扎,1次/天,直至实验结束。于换药第1天、第3天、第5天、第7天、第9天,每次换药开始前选取12只大鼠切取其背部创面中心部位皮肤组织,采用免疫组化及Masson染色结合图像分析的方法检测大鼠创面组织中胶原纤维表达量、Ⅰ型胶原表达量、金属蛋白酶-9及其组织抑制因子-1表达的变化,将取材后的12只大鼠处死,其余大鼠继续实验。结果 换药第1天两组创面组织中胶原纤维、Ⅰ型胶原、MMP-9及其TIMP-1的表达差异均无统计学意义(P>0.05);换药第3天,治疗组创面组织中胶原蛋白及胶原纤维的表达量均高于对照组,但差异无统计学意义(P>0.05);换药第3天直至实验结束,治疗组创面组织内MMP-9含量较对照组降低,TIMP-1含量较对照组升高明显,差异有统计学意义(P<0.05);从换药第5天开始,治疗组创面组织内胶原蛋白及胶原纤维的表达也均高于对照组,差异有统计学意义(P<0.05)。结论 活血生肌方可能通过调节压力性损伤组织中金属蛋白酶-9及其组织抑制因子-1表达水平,从而起到减少胶原蛋白的降解,促进胶原纤维的合成、肉芽组织生长的作用。

      【Abstract】 Objective To observe the effect of huoxue Shengji Recipe on the expression levels of metalloproteinase-9 (MMP-9) and its inhibitor-1 (TIMP-1) in stress damaged tissue of rats, and to explore the mechanism of action. Methods 60 male SD rats were randomly divided into treatment group and control group, with 30 rats in each group. Two stress injury wounds were established on the back of each rat. The treatment group was treated with conventional wound treatment, and the wound was wrapped with sterile gauze after external application of Huoxue Shengji gauze strip, once a day, until the end of the experiment. The control group was treated with routine wound treatment, coated with Vaseline gauze and then wrapped with sterilized gauze, once a day, until the end of the experiment. On the 1st, 3rd, 5th, 7th and 9th day of dressing change, 6 rats in each group were selected to cut the skin tissue of the central part of the back wound before dressing change. Immunohistochemistry and Masson staining combined with image analysis were used to detect the changes of collagen fiber expression, collagen type Ⅰ collagen expression, MMP-9 and TIMP-1 expression in the wound tissue of rats. Twelve rats were killed and the other rats continued the experiment. Results On the third day until the end of the experiment, The content of MMP-9 in wound tissue of the treatment group was lower than that of the control group [(0.381±0.052) vs (0.429±0.061), (0.308±0.035) vs (0.382±0.037), (0.253±0.022) vs (0.328±0.025), (0.187±0.015) vs (0.187±0.015)]. 0.230±0.021), P <0.01]; Compared with the control group, the TIMP-1 content was increased [(3.118±0.632) vs. (2.607±0.572), (4.268±0.518) vs. (3.351±0.617), (5.715±0.745) vs. (4.603±0.685), (7.250±1.521) vs. (6.203±) 1.025), P <0.05]; From the 5th day of dressing change, the expression levels of type Ⅰ collagen and collagen fiber in the wound tissue of the treatment group were higher than those of the control group: [(9.8±2.3) vs. (8.1±1.6), (13.2±3.8) vs. (10.3±2.2), (17.5±4.5) vs. (13.6±3.2); (28.8±7.1)vs.(20.5±6.2), (37.2±7.5) vs. (26.6±6.5), (45.2±8.5) vs.(32.6±7.9), all P <0.05]. Conclusion Huoxue Shengji Recipe may play a role in reducing collagen degradation, promoting collagen fiber synthesis and granulation tissue growth by regulating the expression levels of MMP-9 and TIMP-1 in stress damaged tissues.
