      目的 探讨食物特异性抗体IgG4在早期诊断非IgE介导牛奶蛋白过敏中应用价值。方法 回顾性分析2020年8月至2021年8月杭州市儿童医院消化内科及台州恩泽医疗中心因牛奶蛋白过敏性胃肠病住院的患儿100例为观察组,对照组取同期在杭州市儿童医院及台州恩泽医疗中心因幼儿园健康体检的健康儿童100例,比较2组牛奶蛋白特异性IgE水平和特异性抗体IgG4水平及不同牛奶蛋白IgE分组IgG4水平表达。结果 两组牛奶蛋白IgE水平比较,观察组阳性例数显著高于对照组(c2=23.16,p<0.01)。两组牛奶蛋白IgG4水平比较,观察组显著高于对照组,差异具有显著性(c2=9.86,p<0.01),不同牛奶蛋白IgE分组IgG4水平比较,两组牛奶蛋白IgG4阳性率IgE阴性组显著高于IgE阳性组(c2=12.14,p<0.01)。不同牛奶蛋白IgE分组IgG4水平比较,IgE阴性组显著高于IgE阳性组(t=7.73,p<0.01)。 结论 血清IgG4水平检测有助于早期识别非IgE介导型牛奶蛋白过敏性胃肠病。

      Objective To observe the application of food specific antibody IgG4 in early diagnosis of non IgE mediated milk protein allergy. Methods 100 children ,who hospitalized for milk protein allergic gastroenteropathy in the Department of Gastroenterology in Hangzhou children"s Hospital and Taizhou Enze Medical Center from August 2020 to August 2021 analyzed retrospectively, were enrolled as the observation group; while 100 healthy children who underwent Health examination in kindergarten in Hangzhou children"s hospital and Taizhou Enze Medical Center. Then the levels of milk protein specific IgE, specific antibody IgG4 and IgG4 in different milk protein IgE groups were compared between the two groups. Results The number of positive cases was significantly higher in the observation group than that in the control group (c2 = 23.16, P < 0.01). The level of milk protein IgG4 was significantly higher in the observation group than that in the control group (c2 = 9.86, p < 0.01). The level of IgG4 in different milk protein IgE groups was compared, the positive rate of milk protein IgG4 was significantly higher in the IgE negative group than that in the IgE positive group (c2= 12.14, p < 0.01). The level of IgG4 was significantly higher in IgE negative group than that in IgE positive group (t=7.73, p<0.01). Conclusion The detection of serum IgG4 level is helpful to early identify non IgE mediated milk protein allergic gastrointestinal diseases.
