| 目的 通过对锥形束CT(Cone Beam Computed Tomography,CBCT)成人上颌前磨牙区种植相关骨壁结构的数据进行测量分析,探究该区域牙槽骨高度、厚度与牙位、性别、侧别及是否缺失牙之间的关系,为上颌前磨牙缺失患者种植修复的设计提供参考。方法 选取2019年5月~2022年1月在杭州市中医院口腔科行CBCT检查的上颌前磨牙缺失患者影像资料共63例126位点,其中男33例,女30例,测量其上颌前磨牙区各解剖位点相关数据并进行统计学分析。结果 上颌前磨牙区颊侧牙槽骨最凹处至牙槽嵴顶距离为(9.67±3.81)mm,冠方牙槽骨最凸处至牙槽嵴顶距离为(3.54±1.62)mm,两点中点处至牙槽嵴顶距离为(6.31±2.16)mm,上颌窦底至牙槽嵴顶距离为(14.15±5.07)mm,且在不同牙位、性别与是否缺失牙方面的差异具有统计学意义(P<0.05);骨倒凹深度为(2.21±0.96)mm,在不同牙位、性别与是否缺失牙方面的差异无统计学意义(P>0.05) 结论 上颌骨前磨牙区域的骨壁结构在不同牙位、不同性别等因素下存在差异性,术前应当利用CBCT对术区解剖结构进行测量分析,决定相应的种植方向与深度,以免植体发生颊侧骨壁穿孔,提高种植治疗的成功率。 |

| Abstract Objective To investigate the relationship between height and thickness of alveolar bone and tooth position, gender, side and missing teeth by measuring and analyzing the data of bone wall structure in adult maxillary premolars with Cone Beam Computed Tomography (CBCT). To provide reference for the design of implant restoration in patients with maxillary premolar loss. Methods The imaging data of 63 patients with maxillary premolars missing who underwent CBCT examination in the Stomatology Department of Hangzhou Hospital of Traditional Chinese Medicine from May 2019 to May 2022 were selected, including 33 males and 30 females. The related data of each anatomic site in the maxillary premolars were measured and statistically analyzed. Results The distance from the most concave of the buccal alveolar bone to the top of the alveolar ridge in the maxillary premolar region was (9.67±3.81) mm, the distance from the most convex of the crown square alveolar bone to the top of the alveolar ridge was (3.54±1.62) mm, and the distance from the midpoint of the two points to the top of the alveolar ridge was (6.31±2.16) mm. The distance from the maxillary sinus floor to the alveolar crest was (14.15±5.07) mm, and there were significant differences in different tooth positions, gender and missing teeth (P < 0.05). The depth of bone recessed was (2.21±0.96) mm, and there was no significant difference in tooth position, gender and missing teeth (P > 0.05). Conclusions The bone wall structure of maxillary premolars is different in different tooth positions, gender and other factors. CBCT should be used to measure and analyze the anatomical structure of the operation area before operation. The direction and depth of implant should be determined to avoid the perforation of the buccal bone wall and improve the success rate of implant treatment. |
