      目的 比较高频超声与血清学指标对妊娠期高血压疾病孕妇血管内皮功能检测的差异及相关性。方法 选择2017年9月-2022年10月我院妊娠期高血压疾病孕妇132例作为研究组,选择正常产检产妇132例作为对照组,研究组孕妇进一步分为妊娠期高血压42例、非重度子痫前期64例及重度子痫前期26例。高频超声检测反应性充血前后肱动脉内径ΔD及血流量变化率ΔQ,应用酶联免疫吸附法测定血清可溶性细胞内皮因子(sEng)、可溶性血管内皮生长因子受体(sFlt-1)、内皮素-1(ET-1)、血管内皮生长因子(VEGF)。结果 妊娠期高血压、非重度子痫前期、重度子痫前期组孕妇ΔD及ΔQ明显小于对照组(P﹤0.05),其中重度子痫前期组孕妇ΔD及ΔQ最小(P﹤0.05),非重度子痫前期组孕妇ΔQ值小于妊娠期高血压组(P﹤0.05),但ΔD无统计学差异(P>0.05);与对照组比较,妊娠期高血压、非重度子痫前期及重度子痫前期组孕妇血清sEng、sFlt-1、ET-1明显升高,VEGF明显降低(P<0.05), 重度子痫前期组孕妇血清sEng、sFlt-1、ET-1最高,VEGF最低(P<0.05),妊娠期高血压与非重度子痫前期组孕妇血清sEng、sFlt-1、ET-1、VEGF无统计学差异(P>0.05);超声指标ΔD及ΔQ与血清sEng、sFlt-1呈负相关(P<0.05),与VEGF呈正相关(P<0.05),与ET-1无相关性(P>0.05)。结论 高频超声检测妊娠期高血压疾病孕妇血管内皮功能与血清学指标有较好的一致性,可以进一步推广应用。

      Objective To compare the difference and correlation between high frequency ultrasound and serological indicators in the detection of vascular endothelial function in pregnant women with hypertensive disorders of pregnancy. Methods 132 pregnant women with hypertensive disorders of pregnancy from September 2017 to October 2022 in our hospital were selected as the study group, and 132 women with normal prenatal examination were selected as the control group. The study group of pregnant women was further divided into 42 cases of gestational hypertension, 64 cases of non-severe preeclampsia and 26 cases of severe preeclampsia. The inner diameter(ΔD) and blood flow changes (ΔQ)of the brachial artery before and after the reactive hyperemia thought ultrasound and Serum soluble cell endothelial factor (sEng), soluble vascular endothelial growth factor receptor (sFlt-1), endothelin-1 (et-1), and vascular endothelial growth factor (VEGF) were measured by elisa. Results The ΔD and ΔQ in gestational hypertension, non-severe preeclampsia, severe preeclampsia group were less obviously than the control group (P﹤0.05), in which the ΔD and ΔQ in severe preeclampsia group was the smallest (P﹤0.05), the ΔQ in non-severe preeclampsia group was less than the gestational hypertension group (P﹤0.05), but no statistical difference about the ΔD (P>0.05). Compared with the control group, serum sEng, sFlt-1, ET-1 and VEGF were significantly increased and significantly decreased in gestational hypertension, non-severe preeclampsia and severe preeclampsia groups (P<0.05), Serum sEng,sFlt-1 and ET-1 were the highest and VEGF was the lowest in the severe preeclampsia group (P< 0.05). There was no significant difference in serum sEng, sFlt-1, ET-1 and VEGF between gestational hypertension and non-severe preeclampsia groups (P>0.05). The ΔD and ΔQ were negatively correlated with serum sEng, sFlt-1 (P<0.05), positively correlated with VEGF (P<0.05), and not correlated with ET-1 (P > 0.05). Conclusion The high frequency ultrasonic detection of vascular endothelial function in pregnant women with hypertensive disorders of pregnancy has good consistency with serological indicators, which can be further applied.
