      目的:通过红外线热成像(Thermal Texture Maps,TTM)检测性早熟女童及其早熟类型(中枢性/外周性)的体表热值分布规律,评价其对早期诊断儿童性早熟的预测效能。方法:招募中枢性及外周性性早熟女童各30例、健康女童志愿者60例,比较分析其红外热图特征差异。探讨TTM对女童性早熟的诊断标准,评估其诊断效能。结果:性早熟女童督脉区、膝区体表热值高于健康女童,下腹区、双乳区体表热值离差大于健康女童,差异均有统计学意义(P<0.05)。性早熟女童红外热图典型表现:督脉区呈现沿脊柱走行、连续性高的深红色高温区域,膝区呈现以足三里穴为中心的对称性团块状浅红色高温区域,下腹区及双乳区表现出团块状蓝绿色低温区域或浅红色高温区域。中枢性性早熟女童膀胱子宫区、双附件区、下腹区体表热值高于周围性性早熟女童,具有统计学差异(P<0.05)。中枢性及外周性性早熟女童在膝区、督脉区、双乳区等体表热值无统计学差异(P>0.05)。TTM在诊断是否为女童性早熟准确率为82.5%,诊断区别女童中枢性、外周性性早熟准确率为71.7%。结论:TTM技术能客观反映性早熟女童的体表异常热值差异及其分布规律,其对女童性早熟及其分型具有良好的诊断效能。

      Abstract: Objective: To detect the distribution pattern of body surface heat values in girls with precocious puberty and the subtypes (central/peripheral) by Thermal Texture Maps (TTM) and to evaluate its predictive efficacy for early diagnosis of precocious puberty in children. Methods: Thirty girls with central and peripheral precocious puberty and 60 healthy girl volunteers were recruited to compare and analyze the differences in their infrared thermographic characteristics. The diagnostic criteria of TTM for precocious puberty in girls were investigated, and its diagnostic efficacy was evaluated. Results: The body surface heat values in the governor meridian region and knee region of girls with precocious puberty were higher than those of healthy girls, and the deviations of body surface heat values in the lower abdominal region and bilateral breast region were greater than those of healthy girls, and the differences were statistically significant (P<0.05). Typical infrared thermograms of girls with precocious puberty showed continuous deep red (indicating a higher body surface temperature) in the governor meridian area, symmetrically distributed light red areas in the bilateral calves area centered on Zusanli Point (indicating a higher body surface temperature), and blue-green area or light red area in the lower abdomen and bilateral breasts (indicating a higher or lower body surface temperature).The body surface heat values in the bladder uterine region, bilateral adnexal region, and lower abdominal region were higher in girls with central precocious puberty than in girls with peripheral precocious puberty (P<0.05). The accuracy rate of TTM in diagnosing girls with precocious puberty was 82.5%, and the accuracy rate of diagnosing girls with central and peripheral precocious puberty was 71.7%. Conclusion: TTM technique can objectively reflect the difference of abnormal body surface caloric values and their distribution pattern in girls with precocious puberty, and it has good diagnostic efficacy for precocious puberty and its subtypes.
