      目的:探讨白皮杉醇对大鼠糖尿病心肌病的治疗效果及作用机制。方法:H9c2细胞分为3组:正常组、高糖组(HG组)和白皮杉醇干预高糖组(HG+ Pic组),CCK-8检测各组细胞活性,免疫荧光技术测定H9c2细胞中NF-κB p65的入核率。腹腔注射链脲佐菌素(STZ)建立大鼠Ⅰ型糖尿病(DM)动物模型,分为正常组、DCM组、白皮杉醇低剂量(Pic-L)组[5 mg/(kg·d)]、白皮杉醇高剂量(Pic-H)组[10 mg/(kg·d)],每组各12只。给药12周后,检测各组大鼠血糖,心脏重量/体重比(HW/BW),应用超声心功能检测在体评价各组大鼠心功能,HE染色和天狼猩红染色观察各组大鼠心肌组织病理情况。SOD、MDA试剂盒检测氧化应激反应相关指标水平,TUNEL染色观察心肌细胞凋亡情况。结果:CCK-8结果显示Pic可以提高HG下H9c2的细胞活性(24小时,96.67±5.13%比81.33±6.03%,P>0.05;48小时,102.00±5.57%比84.67±4.93%,P<0.05),细胞免疫荧光染色结果显示Pic降低HG下H9c2细胞中NF-κB p65的入核率。与DCM组大鼠相比,Pic-L治疗组、Pic-H治疗组血糖(19.47±1.21比25.13±1.74 ,P<0.01;13.70±2.18比25.13±1.74,P<0.001)和HW/BW有所下降(4.73±0.68比5.20±0.46,P>0.05;3.70±0.46比5.20±0.46,P<0.05)。相较于DCM组,Pic-L治疗组、Pic-H治疗组大鼠心肌左心室舒张末期直径(LVIDd)和左心室收缩率(LVFS)提高[LVIDd:5.64 ± 0.13比6.50±0.16(P<0.001)、7.41±0.09(P<0.001);LVFS:36.10 ± 0.40比44.83±1.36(P<0.001)、46.77±0.99(P<0.001)],左心室后壁(LVPW)下降[LVPW:1.69 ± 0.05比1.59 ± 0.05、1.52 ± 0.02(P<0.001)],同时心肌细胞形态及排列状态有所改善。SOD、MDA结果显示,对比于DCM组,Pic-L治疗组和Pic-H治疗组的SOD水平提高(59.41 ± 1.98比45.33 ± 3.96,P<0.01;72.30 ± 3.99比45.33 ± 3.96, P<0.001),MDA水平下降(4.19 ± 0.18比5.05 ± 0.26,P<0.01;3.55 ± 0.13比5.05 ± 0.26, P<0.001),能有效缓解氧化应激反应。TUNEL染色结果表明,Pic治疗组能显著降低大鼠心肌细胞凋亡。结论:白皮杉醇可通过抑制心肌氧化应激和炎症反应,减轻心肌细胞凋亡和心肌组织纤维化,防止DCM大鼠的心肌损伤。

      Objective To explore the therapeutic effect and mechanism of piceatannol (Pic) on diabetes cardiomyopathy (DCM) in rats. Methods H9c2 cells were divided into three groups: normal group, high glucose group (HG group) and Pic intervention high glucose group (HG+Pic group). CCK-8 was used to detect cell activity in each group, and immunofluorescence was used to detect the rate of NF-κB p65 nuclear translocation.The rat model of type I diabetes (DM) was established by intraperitoneal injection of streptozotocin (STZ).The rats were divided into normal group,DCM group, Pic-L group [5 mg/(kg·d)], Pic-H group [10 mg/(kg·d)], with 12 rats in each group.After 12 weeks of administration, blood glucose and heart weight/body weight ratio (HW/BW) of rats in each group were detected, heart function of rats in each group was evaluated in vivo by ultrasonic cardiac function detection, and pathological conditions of myocardial tissue of rats in each group were observed by HE staining and Picrosirius red staining.SOD and MDA kit were used to detect the levels of oxidative stress related indicators, and TUNEL staining was used to observe the apoptosis of myocardial cells. Result CCK-8 results showed that Pic could improve the cell activity of H9c2 under HG conditions (24h,96.67±5.13% vs 81.33±6.03%,P>0.05;48h,102.00±5.57%vs84.67±4.93%,P<0.05),and the results of cell immunofluorescence staining showed that Pic reduced the rate of NF-κB p65 nuclear translocation. Compared with DCM rats, the blood glucose(19.47±1.21 vs 25.13±1.74,P<0.01;13.70±2.18 vs 25.13±1.74,P<0.001)and HW/BW (4.73±0.68 vs 5.20±0.46,P>0.05;3.70±0.46 vs 5.20±0.46,P<0.05) in Pic-L and Pic-H treatment groups decreased. Compared with the DCM group, the left ventricular end-diastolic diameter (LVIDd) and left ventricular systolic rate (LVFS) of rats in in Pic-L and Pic-H treatment groups increased [LVIDd:5.64 ± 0.13 vs 6.50±0.16(P<0.001)、7.41±0.09(P<0.001);LVFS:36.10 ± 0.40 vs 44.83±1.36(P<0.001)、46.77±0.99(P<0.001)], and the left ventricular posterior wall (LVPW) decreased [LVPW:1.69 ± 0.05 vs 1.59 ± 0.05、1.52 ± 0.02(P<0.001)], at the same time, the morphology and arrangement of myocardial cells were improved. The results of SOD and MDA showed that compared with DCM group, the level of SOD in Pic-L and Pic-H treatment groups increased(59.41 ± 1.98 vs 45.33 ± 3.96,P<0.01;72.30 ± 3.99 vs 45.33 ± 3.96, P<0.001)and the level of MDA decreased(4.19 ± 0.18 vs 5.05 ± 0.26,P<0.01;3.55 ± 0.13 vs 5.05 ± 0.26, P<0.001), which can effectively alleviate the oxidative stress reaction. TUNEL staining results showed that Pic treatment group could significantly reduce myocardial cell apoptosis in rats.Conclusion By inhibiting myocardial oxidative stress and inflammatory reaction, Pic can reduce myocardial cell apoptosis and myocardial fibrosis, and prevent myocardial injury in DCM rats.
