| 目的：鼻部淋巴瘤患者中常发生延迟诊断、误诊和误治，通过本文学习，提高其诊疗及时性及正确率。方法：本研究收集2010年1 月至2020 年9 月在浙江省台州医院经病理证实的淋巴瘤患者54例，进行回顾性分析，探讨了鼻部淋巴瘤的临床表现和诊断。结果：鼻部淋巴瘤的临床表现主要有三种。第一种表现为鼻面部水肿，可累及眼部，出现眼睑肿胀、溢泪、眼球突出、视力下降或视野缺损为突出表现。第二种表现为整个鼻腔甚至鼻咽部，表面粘膜糜烂，表面有干痂，甚至发展为硬腭穿孔，通常伴有鼻味。最后一种是早期鼻部淋巴瘤较常见的表现，可见鼻腔局部肿胀，变白，或者侵蚀位于前鼻腔，有时仅位于下鼻甲。结论：对于治疗效果差，其鼻内镜检查鼻腔或鼻咽部有糜烂、溃疡（坏死）、结痂、周围软组织水肿或有鼻腔新生物的患者，应认真考虑鼻部淋巴瘤的可能性，应及时进行病理活检以确诊。 |

| Misdiagnosis, delayed diagnosis and mistreatment often occur in patients with nasal lymphoma.In this study, the clinical manifestation and diagnosis of nasal lymphoma is discussed, based on a retrospective study of 54 casesdiagnosed in Taizhou Hospital of Zhejiang Province from January 2010 to September 2020. There are three kinds of clinical expression. One is nasofacial edema,eyelid swelling, tears, exophthalmos, decreased vision or visual field defect. Another is erosion, uler and scab diffuse distribution in the nose, even progressing to hard palate perforation, usual with nasal odor. The last is the manifestation of early nasal lymphoma, swelling, whitening maybe erosion locating at anterior nasal cavity, sometimes at inferior turbinate only. For patients with poor therapeutic effect and the above expressions, the possibility of nasal lymphoma should be seriously considered, and timely pathological biopsy should be performed to confirm the diagnosis. |
