| 肿瘤内科治疗从传统化疗逐步向化疗联合免疫、靶向的精准治疗方式转化,在延长肿瘤患者生存期的同时,靶向治疗不可避免地存在一些毒副作用,成为制约肿瘤后续治疗的主要因素。通过观察临床靶向治疗不良反应,发现其具有影响多系统的特征,病位累及三焦,涉及三焦腠理病变,影响三焦气液传导输送功能失司。结合中医理论认识,靶向药毒致病属性与卫阳浮亢相关,表现出风热袭络的病理特征,药毒侵虚是肿瘤患者靶向治疗后出现不良反应的最主要病机,因此在治疗原则上既要清除药毒,防控进一步药毒损伤,也需固护本元,维持机体基本生化。具体治疗方式当以疏利三焦,清除药毒为先,调节营卫,平复亢阳治其标,补益三焦,扶正祛邪治其本。 |

| Medical oncology treatment has gradually transformed from traditional chemotherapy to chemotherapy combined with immune and targeted precision therapy. While prolonging the survival of tumor patients, targeted therapy inevitably has some toxic and side effects, which has become the main factor restricting the follow-up treatment of tumors. Through the clinical observation of the adverse reactions of targeted therapy, the adverse reactions show a variety of symptom characteristics in various systems. The location of the disease involves the pathological changes of the interstice and the loss of the gas-liquid transmission function of the sanjiao. The pathogenic attribute of targeted drugs is related to the hyperactivity of defense qi, showing the pathological characteristics of wind-heat attacking the collaterals. Targeted drug toxicity invading debility is the main pathogenesis of adverse reactions in tumor patients after targeted therapy. Therefore, in the principle of treatment, it is necessary to clear the drug toxin, prevent and control further damage caused by the drug toxin, and also need to protect the body and maintain the basic biochemistry of the body. The treatment method should focus on dispersing the Sanjiao, clearing the drugs and poisons first, harmonizing the nutritive and defense, calming the hyperactive yang, treating its symptoms, nourishing the Sanjiao, strengthening the vital qi to eliminate pathogenic factor and treating the root cause. |
