| 【摘要】目的：探讨螺钉内固定对喙突骨折的治疗效果和体会。方法：采用回顾性病例研究分析2018年7月-2023年1月在杭州师范大学附属医院收治的7例喙突骨折的临床资料，其中男5例，女2例；年龄27～73岁，平均49.8岁。Ogawa 分型：7例均为I型；Eyres分型：III B型1例，IV A型1例，IV B型1例，V A型1例，V B型3例。1例单纯喙突骨折，2例合并肩锁关节脱位，1例合并肩峰骨折，2例合并锁骨骨折，1例合并肩峰、锁骨骨折。喙突骨折均采用螺钉内固定。记录喙突骨折手术时间、术中出血量、骨折愈合时间、肩关节Constant-Murley评分和Herscovici评分。观察术后并发症情况。结果：患者随访3~36个月，平均18.6个月。手术时间平均110.7min、术中出血量平均51.4ml。骨折愈合时间平均14.6周。Constant-Murley评分为63～100分，平均82.6分，Herscovici评分为9～16分，平均13.4分。术后均未出现感染、神经血管损伤和内固定松动等并发症。结论：对于喙突骨折，掌握正确的置钉和透视技巧至关重要，螺钉固定具有手术创伤较小、固定坚强、并发症少、肩关节功能恢复满意等优点。 |

| 【Abstract】 Objective: To discuss clinical experience for screw fixation in the treatment of coracoid process fracture. Methods: 7 patients of coracoid process fracture were selected with screw fixation from July 2018 to January 2023. There were 5 males and 2 females, whose average age was 49.8 years old, ranging from 27 to 73 years old. According to the Ogawa classification, all the patients were type I. According to the Eyres classification, there were 1 case of type III B, 1 case of type IV A, 1 case of type IV B, 1 case of type V A, 3 cases of type V B. Coracoid process fractures of all the patients were treated with screw fixation. The clinical indicators including operation time, haemorrhage, bone union time，range of motion of shoulder joint were recorded. Constant-Murley criteria and Herscovici criteria were used to evaluate function of shoulder joint. Results: All the cases were followed up for 3~36 months, averaged 18.6 months. The mean operation time was 110.7min, The mean haemorrhage was 51.4ml. The mean time of bone union was 14.6 weeks, ranging frome 8 to 28 weeks. Average Constant-Murley score was 82.6, ranging from 63 to 100. Average Herscovici score was 13.4, ranging from 9 to 16. There were no postoperative complications. Conclusions: For coracoid process fracture, correct screw placement and fluoroscopy techniques are crucial, which can provide less surgical trauma, strong fixation. Satisfactory clinical effect all can be obtained. |
