| 【】 目的 探讨子宫破裂的发生率、高危因素、临床特点及早期预防。方法 回顾性分析绍兴市妇幼保健院2014年1月至2022年12月收治的17例子宫破裂患者的临床资料。结果 子宫破裂17例,其中子宫不全破裂6例,均为剖宫产后原疤痕处破裂,母儿结局良好。完全子宫破裂11例,因异位妊娠手术治疗后5例,胎盘植入致子宫破裂2例,剖宫产原疤痕破裂2例,子宫肌瘤剔除部位破裂1例,残角子宫妊娠破裂1例。以下腹痛为首发症状9例,超声协助诊断8例,输血4例,残角子宫切除1例,其余无子宫切除,2例死胎,1例重度窒息放弃抢救,其余8例新生儿结局良好。 结论 疤痕子宫再次妊娠应视为发生子宫破裂的高危人群，此类患者再次妊娠时需全面评估，详细了解既往子宫手术史，并做好高危门诊管理，早诊早治，尽可能减少子宫破裂及严重妊娠并发症的发生。 |

| Objective To investigate the incidence，high risk factor，clinical characteristics and early prevention . Methods The clinical data of 17 patients with uterine rupture admitted to Shaoxing Maternal and Child Health Hospital from January 2014 to December 2022 were retrospectively analyzed。 Results The incidence of uterine rupture was 0.018%，among them, 6 cases were incomplete rupture, all of which were rupture at the original scar after cesarean section； Complete uterine rupture occurred in 11 cases, including 9 cases with previous uterine operation history，there was one case of pregnancy with residual horn uterus，the residual horn uterus was resected during the operation, and the uterus was preserved successfully in other patients，2 cases were stillborn , 1 case was severely asphyxiated and the other 8 cases had a good outcome。Conclusion Scarred uterus in recurrent pregnancy should be considered as a high-risk population for uterine rupture. Such patients require comprehensive assessment and detailed understanding of their previous uterine surgical history. Furthermore, meticulous high-risk outpatient management should be implemented, aiming for early detection and treatment, in order to minimize the occurrence of uterine rupture and severe pregnancy complications. |
